Job description

Main Purpose of the Post To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. To work as a member of the general practice nursing team, to support the organisation and facilitation of clinical care in a holistic and proactive manner to patients registered at the practice. Responsible to: GP Partner Nurse Lead for Clinical Responsibility Practice Manager for Administrative purposes Special Requirements of the Post Able to perform venepuncture. An enthusiastic, proactive and positive individual who is able to work well within the clinical team.

Have an understanding, acceptance and adherence of the need for strict confidentiality and clinical governance. Flexible to the service needs and be able to adapt times and location of working, increasing hours if needed, to cover for holiday and sickness. Main Duties and Responsibilities of the Post This will include, but are not exclusive to: Ensure that the patient is as comfortable and relaxed as possible and consents to the collection of a blood sample. Check patient identification against electronic records and blood sampling request form before any sample taken.

Collection of samples of blood in accordance with training and procedure. Accurate labelling of samples to confirm identity. Ensure samples are stored in appropriate containers. Ensure samples are dispatched to the correct laboratories in a timely fashion.

Correct disposal of Sharps. Take and record blood pressure and pulse rate. To chaperone, as requested by clinicians. Assist GPs with clinical procedures such as minor surgery.

Ensure that any queries or patient issues are discussed with GP Partner Nurse Lead or Practice Manager. Ensure up to date with any mandatory training required of the post. Maintain general housekeeping such as recording fridge temperatures and stocking of clinical rooms. Any other clinical duties within your remit that you are required to undertake after appropriate training has been provided.

Administrative and Professional Responsibilities Participate in the administrative and professional responsibilities of the practice team. Ensure accurate notes of all consultations and treatments are recorded in the patients computerised medical record. Ensure accurate completion of necessary documentation associated with patient health care and registration with the practice. Attend and participate in all practice meetings as required.

To undertake all administrative procedures relating to the post, liaising with the Managing Partner/Practice Manager. To co-operate and participate in the collection of statistics for audit projects. Confidentiality Under the Data Protection Act 2018 (alongside the EU General Data Protection Regulations) the postholder must maintain the confidentiality of information about patients and staff.

The work is of a confidential nature and information gained must not be communicated to other persons except in the recognised course of duty. Unauthorised disclosure of confidential information may result in disciplinary action and may lead to your dismissal. Contribution to the Implementation of Services The post-holder will: Apply practice policies, standards and guidance. Discuss with other members of the team how the policies, standards and guidelines will affect own work.

Participate in audit where appropriate. Health & Safety Employees must be aware of the responsibilities placed on them under the Health & Safety Act at Work (1974) to ensure that agreed procedures are carried out to maintain a safe environment for patients, visitors and staff, including infection prevention and control. The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety Policy, to include: Use personal security systems within the workplace according to practice guidelines. Keep own work areas and patient areas generally clean Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements and good practice guidelines. Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements.

Adherence to their own individual hand hygiene Adherence of current working practices across the practice in relation to infection control, cleanliness and related-activities, ensuring that procedures are followed and issues escalated to the either the GP Partner Nurse Lead or Practice Manager. Waste management including collection, handling, segregation, container management, storage and collection. Identify the risks involved in work activities and undertaking such activities in a way that manages those risks. Demonstrate due regard for safeguarding and promoting welfare of adults and children.

Make effective use of training to update knowledge and skills. Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards. Reporting potential risks identified. Personal/Professional Development The post-holder will participate in any training programme implemented by the practice as part of this employment, such training to include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.

Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work. Quality The post-holder will strive to maintain quality within the practice, and will: Alert other team members to issues of quality and risk. Assess own performance and take accountability for own actions, either directly or under supervision. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance.

Work effectively with individuals in other agencies to meet patients needs. Effectively manage own time, workload and resources. Rehabilitation of Offenders Act This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions. Safeguarding To be fully aware of and understand the duties and responsibilities arising from the Childrens Act 2004 and Working Together in relation to child protection and safeguarding children and young people as this applies to the workers role within the organisation.

To also be fully aware of the principles of safeguarding as they apply to vulnerable adults in relation to the workers role, which will include recognising the types and signs of abuse and neglect and ensuring that the workers line manager is made aware and kept fully informed of any concerns which the worker may have in relation to safeguarding adults and/or children.
