Job Description - Primary Care Network Liaison Manager
Responsible to - FedNet Operational Lead, Clinical Lead and the Board
Accountable to - The Board
Band 6 – 7 (Depending on experience)
Location - WF FedNet Headquarters, Chingford, Waltham Forest
Contract - Fixed Term up to 9 months with opportunity of extension
Job Summary
This is a new role connecting WF Federation with member practices as Primary Care Networks (PCNs) develop and mature. These networks are practices that work together within a geographical area to deliver contracts and services for their GP-registered population.
The post holder will be responsible for supporting the operational management and development of the respective PCNs.
The PCN Liaison Manager will form an integral part of the multiple PCN leadership working alongside the Medical Director and the board members.
The post holder will act as the main point of contact of expertise, help coordinate the successful achievement of all PCN DES contractual requirements, support the translation of national and local policies into meaningful changes, and support collaborative working between multiple PCN organisations and the Federation to achieve improved outcomes for the local population.
The post holder will be responsible for strategic oversight, keeping abreast of all changes likely to affect the service now and in the future; including the operational planning, designing, implementing and reviewing new services and day-to-day management of these, for the PCN.
The post holder will support the development, management and maintenance of systems and processes to ensure all engaged PCNs operate at the highest level of performance and accountability.
MAIN DUTIES AND RESPONSIBILITIES
The post holder will:
a. Be a point of contact for the Practices with regard to the delivery of PCN projects/schemes across engaged PCNs.
b. Analyse service specification, performance indicators and data collection requirements to improve the design of new PCN services and help offer consistency across the borough
c. Support service delivery by being the conduit between the Federation and the PCN and its member practices, thereby ensuring integration of the new service staff into existing teams and working processes
d. Develop metrics and other arrangements to support the subcontracting of services and subsequent contract management
e. Support the management of operational processes of the PCN and for the development of Standard Operating Procedures for PCN services, ideally across many PCNs
f. Help manage/understand financial requirements on behalf of the PCNs
g. Support and/or coordinate training and development of PCN clinical/administrative staff
h. Help develop joint working and relationships across networks and its stakeholders.
i. Maintain relationships across all core member Practices
j. Build external relationships and identify opportunities to work with wider system partners
k. Drive continuous improvement across the PCNs
l. Develop and maintain all PCN databases and policies
m. Carry out any other duties as may reasonably be required by the Clinical Director, Board and its members.

Person Specification
· Educated to degree level or equivalent significant experience
· Formal Management Qualification or equivalent significant experience
· Understand the payment structures within primary care
· Knowledge of QOF, NHS long-term plan and Enhanced Services
· Hold a management qualification appropriate to the post.
· Hold a project management qualification appropriate to the role
EXPERIENCE
· Experience in a senior role working within Primary Care or similar with transferable skills
· Experience in Practice management (preferably as practice manager or assistant practice manager)
· Clear evidence of managing financial accounts procedures and budget control
· Experience of working on GP clinical systems
· Experience of developing and implementing common systems and processes across teams and across service boundaries
· Experience of facilitating and managing change
· Experience in managing and leading staff
· Experience in managing records and data protection issues
· Project Management experience including QI methodology
