Job Title: Patient Care Coordinator
Responsible to: Operations Manager
Based at: Kingsteignton Medical Practice
Job Summary: A Care Coordinator will support the clinical teams by performing administration tasks, freeing up clinical time. Our aim is to provide exemplary patient care; finding innovative solutions in general practice to deliver the best care we can to our patients.
Key Result Areas:
- Processing and filing all correspondence received in practices and prioritising for GPs in terms of actions, safety netting against potential delays due to workflow demand.
- Signposting correspondence to members of the practice multi-disciplinary teams (MDT) as appropriate, such as clinical pharmacist etc.
- Identifying and extracting all relevant information and actions from clinical correspondence received.
- Data entry of relevant information from incoming correspondence, medical coding and filing on to patient medical records (e.g., diagnosis, investigations, results, history, medication etc.).
- Improving practice efficiency and accuracy of data collection. Through the consistent, accurate application of medical coding and collection of numerical values (e.g., blood pressures, height, and weight). GPAs will ensure medical records will have the accurate and relevant information required to deliver high quality patient care and monitor practice performance through the NHS Quality and Outcomes Framework (QOF) to ensure we maintain service delivery.
- Facilitating actions outlined in correspondence received, e.g., scheduling appointments, referrals, tests and follow up appointments of patients in a timely manner.
- Support the practices in identifying appropriate patient cohorts for targeted intervention.
- Work with practices to support delivery of any national and local targets with regard to the
GP contract e.g., Network Contract Direct Enhanced Service (DES).
- Undertaking quality improvement and change in practice. Assist with audits as required
- To support the Clinicians as a GP Buddy:- o liaising with patients thought not to be appropriate for the team and needing an alternative plan/appointment o Booking in patients who they know need a face-to-face visit without a call first o Following-up with previous patients they want to keep an eye on, either by checking records, hospital reports etc or phoning the patient directly o Calling patients to get more information to enable the team to prioritise effectively
- To help identify patients/or work with patients upon request by the clinicians that are ending up on their list due to a lack of home support/poor management of their condition
that need additional interventions which could include proactive calls from you, or involving the wider clinical team
- To help identify patients that may benefit from additional external/non-clinical support, such as Social Prescribers
- Organising & attendance at MDT meetings
- Taking of notes at MDT meetings & preparation of associated minutes
- Act as a role model within the Patient Services Team demonstrating support and motivation
- Promote positive relationships and good teamwork.
- Attend relevant meetings as required including staff meetings. You may be requested to attend other meetings at the request of the medical or management team.

Collaborating Working Relationships
- Work collaboratively with the PCN team
- Develop relationships and work closely with community, hospitals, pharmacy colleagues and the wider health system
- Liaise with other stakeholders for the collective benefit of patients
- Attend relevant network and practice meetings

Person Specification

Academic/Vocational GCSE grade A to C in English and Maths

Salary: £10.65-£12.92 per hour
