Form Health (https://www.formhealth.co) is telehealth for adults with obesity, a chronic excess weight disease.
Obesity impacts more than 40% of the US adult population, but only about 1% of patients receive medical treatment for their disease. Form Health makes research-based treatment available to adults through the convenience of telehealth, including long-term care from a physician and dietitian and medication when appropriate.
Founded in Boston in 2019 with a public launch in early 2020, Form Health is a venture-backed startup with an experienced team and product-market fit. We’re building for growth: onboarding more patients, building better tools for our clinicians and partners, and scaling our team while maintaining a high bar for patient success.
About the role:
Form Health Patient Care Coordinator(s) provide comprehensive support for daily clinical operations, and growth of the clinical practice and the company. We and our patients need your support to provide an outstanding experience to every single patient we serve and increase practice efficiency. Working collaboratively with the physician and dietitians in our practice, you will help us to scale by developing, organizing, and supporting processes that enable excellent patient care as well as clinical growth. We are passionate about helping people improve their health through weight loss, and will provide you opportunities for learning and professional growth.
Form is accepting candidates living in Boston Area. There is currently flexibility to spend time working remotely but will require 3 days in office.
Where you’ll deliver impact:
We seek an experienced candidate, a team player with exceptional interpersonal and organizational skills, who is passionate about being part of a health-care team and excited to work in a non-traditional telemedicine practice environment.
Key responsibilities include:
- Receiving, and triaging or resolving, incoming clinical messages from patients to clinicians. Also relaying clinical messages from clinicians to patients when requested.
- Scheduling and rescheduling new and returning patient appointments for physicians and dietitians, as well as coordinating clinician schedules/availability. Coordination of appointments requires close attention to detail, and some flexibility with work schedule can be helpful for connecting with busy patients.
- Registering and onboarding new patients. This includes collection of patient information, reaching primary care or other providers for medical records, sending patients welcoming communications and equipment, creating new patient records in the medical record and uploading consent forms and outside records, sending lab requisitions, as well as any other duties as needed to enroll new patients to our practice.
- Regular outgoing patient communications to ensure they remain consistently engaged with our clinical team, which helps to optimize their success in the program.
- Helping with prior authorization and other prescription-related tasks.
- Answering incoming company phone calls. Helping answer or identify answers to questions by contacting the appropriate person and getting back to the caller as necessary to ensure customer satisfaction.
- Providing routine administrative support for clinicians, such as assisting with scanning, copying, filing, faxing or texting as required for patient care.
- Providing feedback and communicate patient feedback to other company teams (i.e. Product, Engineering, Marketing) to improve operational tools and patient care.
- Assisting with clinical data collection as needed to track company’s key clinical performance indicators or for quality improvement purposes.
- Assisting with scheduling of clinical staff meetings.
- Assisting with training and orienting new staff.
- Assisting with special projects as directed.
You’ll be successful in this role because you have:
- Minimum of 3 Years of Patient Coordinator or Medical Assistant experience required , preferably in bariatric/weight-loss clinic.
- Applicants working in tele-health, multi-state practice preferred.
- Strong customer service drive, a passion for helping patients, and with efficient work flows.
- Excellent English language skills. Excellent verbal and written communication skills.
- Strong organizational, coordination and judgment skills. Ability to prioritize effectively. Ability to manage multiple tasks effectively. Ability to manage time effectively.
- Advanced computer skills. Will be working with our custom-built and evolving IT systems.
- Minimum of a college degree.
- Must live in Boston, MA area.
