Job summary

We have an exciting opportunity for two enthusiastic Paramedics to join our dynamic team. This permanent position is well suited to individuals who are enthusiastic about providing high quality support to primary care.

Based within Medway Peninsula PCN, you will work closely with GP Practices and Care Homes within the primary care network. GP Practices include Highparks Medical Practice, The Elms Medical Practice and St Werburgh Medical Practice in Rochester, Kent.

You will be joining forces with our established paramedic and a team of clinicians within the PCN, This includes Pharmacists, First Contact Physiotherapists, Physicians Associates, and Social Prescribers.

Although your working days and hours will be standard, your work patterns are likely to vary and may consist of home visiting as well as providing clinics in GP practices seeing patients with a variety of conditions. Your allocation of patients will be according to your clinical skill set/competencies and following a triage process to ensure patients are allocated to the appropriate clinician.

Main duties of the job

KEY RESPONSIBILITIES

  • Assessing and treating patients presenting with minor or acute illnesses in their own place of residence and at a GP practice.
  • Triage and treat patients, making any necessary referrals to other members of the primary health care team or organise admission if required.
  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations

KEY REQUIREMENTS

  • Active and relevant professional registration with HCPC
  • Evidence of relevant CPD

Job summary:

  • The role is for a Paramedic to support primary care teams by providing home visiting services and practice based medical services during the working day, Monday to Friday.
  • The clinician will work as part of the GP practice team with close collaborative working with other members of the Multidisciplinary Team and support from GPs.
  • To work within the community, as an autonomous, accountable Paramedic.
  • To provide holistic care for individuals including assessment, diagnosis and management
  • To refer and/or signpost patients in their own home/care home or at the practice to other allied health professionals for example community COPD nurses, Social prescribers etc.
  • The post holder will use advanced clinical and communication skills to provide education to patients, promoting self-care and empower them to make informed choices about their treatment.
  • Care occasionally may need to be provided in other settings as required.

Essential

  • Ability to manage and prioritise workload.

  • Basic IT skills in word processing and e-mail.
  • Record keeping skills.

Experience

Essential

  • Experience of working within a multi-disciplinary team
  • Experience in using I.T. based patient documentation systems.
  • Evidence of experience / competency in the form of a professional portfolio.
  • Able to professionally and appropriately challenge differing opinions as a patient advocate.

Desirable

  • Experience of clinical supervision
  • Experience of working in Primary and Community Care

Circumstances

Essential

  • Car owner/driver, with exclusive access to a vehicle
  • Ability to work alone in the primary care setting

Knowledge

Essential

  • Understanding of health education/promotion.
  • Understanding of Confidentiality and the Data protection Act
  • Understanding of clinical governance and its significance and impact upon practice.
  • Ability to participate in audit and standard setting

Qualifications

Essential

  • Active and relevant professional registration with HCPC
  • Evidence of relevant CPD

Desirable

  • BSc in a training programme approved by the College of Paramedics
  • Advanced Life Support Provider
