We have an exciting role available within our Intermediate Care team as a band 6 Occupational Therapist.
Enhanced Recovery and Support at Home is part of Frimley Health’s Intermediate Care Service. Providing holistic assessment and case management for admission avoidance and to facilitate discharges from hospital, you will gain additional extended skills. We are a large but close MDT with senior clinical leadership. You will be supported develop and release your potential by one of the three B7 OTs, undertaking CPD opportunities and receiving regular 1:1s and caseload supervision.
This is a fast paced team with a variety of opportunities to develop areas of special interest. You will be given the opportunity to develop your own leadership and management skills, supervising junior staff and supporting students on placement.
Key Responsibilities:
1. To provide holistic OT assessment, goal setting, individualised treatment programmes, care planning and delivery.
2. Regular review and discharge of patients.
3. To manage a varied caseload, seeking support from senior staff where necessary to ensure the provision of a timely, clinically effective service based on evidence based practice.
4. To provide services to support discharge of patients from acute hospital and to provide an admission avoidance service for patients in crisis within community settings.
5. To undertake in-reach to acute and community settings.
6. To be actively involved in, and lead on, projects towards the development & delivery of the service and to contribute to new developments and initiatives.
7. To provide standardised cognitive assessments and treatment programmes as required.

1. Where appropriate to take responsibility for the application of new skills to rehabilitation programmes
2. To provide line management and supervision to junior staff and practice placement students as appropriate
3. To maintain excellent clinical practice through the CPD process, and taking part in relevant research, audit and development activity as directed
4. To work closely and jointly at times with Physiotherapy and Nursing colleagues to ensure the patient receives holistic treatment
5. To be responsible for the day-to-day management and prioritisation of your caseload and be able to respond to urgent referrals and at times an unpredictable workload. This could require prolonged concentration in an unpredictable environment
6. To be able to manage potentially stressful, upsetting or emotional situations in an empathetic manner when imparting bad news regarding rehabilitation potential. To ensure this is communicated sensitively to all parties concerned
7. To keep yourself well at work both physically and emotionally, to access peer, clinical and management support when needed and to be aware of own limitations and escalate accordingly
8. To report risks and harm to patients using the Trust reporting system
