JOB DESCRIPTION
JOB TITLE: Nurse Practitioner
REPORTS TO: The Partners (Clinically)
The Practice Manager (Administratively)
HOURS: 37.5 hours per week
Job Summary:
To provide leadership and supervision to the practice nurse team in order to develop an effective, quality driven practice nurse service in the provision of assessment, treatment, screening, health education and advice to patients.
To promote integrated nursing practice within the wider nursing team (Practice Nurses, District Nurses and Health Visitors) underpinned by the development of evidence based practice guidelines, for shared delivery of care to the whole practice population.
To work as part of the primary healthcare team providing personal medical services to the patients of the practice.
To provide clinics for patients who wish to be seen by a health care professional; diagnosing and treating patients presenting with minor illness; seeing and advising people in respect of their continuing medical and nursing needs by prioritising and triaging the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
To see extra or emergency patients if required.
To visit patients at residential addresses in liaison with GPs.
To work in accordance with the NMC code of conduct.
Roles and Responsibilities:
Leadership and supervision responsibilities:
- Provide leadership and supervision to the nursing team to enable them to provide a high quality service to patients and to work effectively with other agencies
- To liaise with the Nurse team leader/practice manager to identify and recruit the workforce that is required to provide services to meet the needs of the patient population
- Promote clinical effectiveness in the nursing team by applying and improving clinical governance and risk management systems through key quality markers
- Liaise with the Nurse team leader/practice manager to development and monitor implementation of policies, standards and guidelines across the practice nursing team
- Analyse, interpret and report data on health and well-being, related needs and service use
- Allocate, co-ordinate, monitor and assess the work of the nursing team and individuals
- Liaise with the Nurse team leader/practice manager to supervise and assess staff performance and development including annual appraisal
- Offer and provide clinical supervision to other members of the nursing team and act as a clinical ''expert'' appropriate to level of competence
- Develop a working environment that promotes healthy, safe and effective work outcomes
- Identify training needs of the nursing team
- Ensure skills of practice nursing team are appropriately and effectively used to deliver care
- Monitor the nurse appointment system, in collaboration with NTL/practice manager
- Supervise the nursing team in the maintenance of equipment and stock control
- Organise and coordinate the provision of nursing services for the practice to include the delegation of duties to other members of the practice nurse team as appropriate
- Plan, develop, implement and evaluate new health promotion and chronic disease management services
- Provide professional advice on Practice developments, recommending changes to working practices in line with evidence based knowledge.
- To liaise with reception, office and administration staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person and ensuring every one is aware of the different roles within the nursing team
- Establish and maintain good liaison with surgeries and agencies including secondary care
Training and personal/professional development
The post-holder will participate in any training programme implemented by the practice as part of this employment, such training to include:
- Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development
- Participation in in-house or collective training sessions
- Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work. It is the individuals’ responsibility to remain up to date with recent developments
- Training requirements will be monitored by yearly appraisal
- Personal development will be encouraged and supported by the practice.
- Participate in the education and training of students of all disciplines and the induction of all members of the practice staff where appropriate
- Develop and maintain an in house nurse-training programme reporting to the practice manager
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring PREP requirements are met
- If it is necessary to expand the role to include additional responsibilities, training will be given
- Develop and maintain a Personal Learning Plan
Manage, supervise, co-ordinate and guide the nursing team
- To assist in the management of the nursing team in participation with the practice manager ensuring adequate training is provided and relevant information is disseminated to and from the nursing staff
- In participation with the NTL/practice manager carry out yearly appraisals of the nursing staff
- Ensure adequate nursing cover, keeping the practice manager informed of any problems
- Assist in the development and maintenance of practice policies and protocols for nursing
- Support for practice infection control by ensuring all staff are aware of current procedures
- Assist the NTL plan and minute management and training meetings for all nurses
- Ensure agreed practice audits are carried out
- Ensure the practice manager is informed of any matters arising or potential problems
- Ensure all surgery produced nursing leaflets are kept up to date
Administration
- Participate in the administrative and professional responsibilities of the practice team
- Ensure appropriate items of service claims are made accurately, reporting any problems to the practice manager
- Collection and maintenance of statistical information required for regular and ad hoc reports and audit including QoF, Prescribing data and any other information required
- Attend and participate in Practice meetings as required
- Supervise the restocking and maintenance of clinical areas and consulting rooms
- Supervise the stock and re-ordering of medical supplies including injections and equipment
- Provide a vehicle with suitable insurance cover to allow home visits to be made
General clinical practice:
- Practice independently in accordance with the NMC code of conduct and with reference to practice guidelines, appropriately referring to GPs
- Maintain that competence by attending annual updates, and refer to current BNF, SPCs PGD’s and GP and CCG guidance
- Ensure continuation of own personal and professional development, facilitated by reflective practice and in house clinical supervision
- Maintain accurate documentation of care and utilisation of I.T and ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
Diagnosing and treating patients presenting with minor illness
- Triage and treat patients wishing to see a health care professional making any necessary referrals to other members of the primary health care team
- Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
Prescribing:
- Utilise qualification as Nurse Independent and Supplementary Prescriber (NISP) to prescribe for practice patients within the limits of current PCT prescribing guidelines and own professional competence and accountability
- Maintain that qualification by attending annual updates and personal and professional development and refer to current BNF, SPCs PGD’s and also GP and PCT guidance
Management of Chronic Disease
- Utilise specialist qualifications in Asthma, COPD, Diabetes and Family Planning to benefit the practice population directly in clinics
- Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
- Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach
- Identifying the impact of treatment and implementing or recommending changes as appropriate
- Liaising with outside agencies where more specialist input is required, e.g. Specialist Nurses, Health Protection Agency (HPA) or Travax
Patient Health Checks , Health Screening and Risk Assessment
- Provide general and specific health screenings to patients (within agreed protocols) with referral to other members of the primary and secondary health care team as appropriate
- Identifying significant abnormalities
- Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues
- Working with patients to develop a management plan where health problems or potential health problems are identified
- Recognising issues and gathering sufficient information to refer (e.g. women’s and men’s health, sexual health, older people)
- Undertaking monitoring tasks (e.g. smears) and providing advice as appropriate
- Providing in depth monitoring and advice as appropriate to the level of the post-holder’s specialty and working with the patient in deciding on management plans.
- Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems)
Travel Health
· Supervise the nursing team in travel health care
· Establishing the patient’s immunisation status
· Identifying health status/medical complications
· Carrying out a risk assessment of the proposed travel with the patient
· Signposting the patients to evidence based travel health resources for self assessment
· Administering injections and providing evidence based guidance
Immunisation (Adult and Child)
- Administering appropriate immunisation autonomously
- Working with patients with more complex immunisation issues.
- Liaising with Health Visitors where relevant
Supervision of the nursing team in providing assessment, screening treatment services and health education advice:
- Dietary advice
- Blood pressure checks
- Urinalysis
- Health screening
- Ear syringing
- Dressings
- ECG
- Phlebotomy
- Routine injections
- Contraception
- Minor injuries
- Routine immunisations and vaccinations
- Removal of sutures / clips
- Baby immunisations
- Assisting with the fitting of IUCD / IUS
- Health checks / health surveillance / general health advice
- Assisting with minor procedures in the treatment room
Pathological specimens and investigatory procedures
- Train and supervise the nursing team in the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs
Minor Operations
- Management of the preparation for, undertaking and clearing up after minor operations including maintenance of equipment and stock, adequate staffing and ensuring appropriate items of service claims are made
- To ensure the doctor has the required support to carry out minor operations as required
- Report any problems to the Practice Manager
Home Visits
Triage all home visit requests at the end of each morning session.
Book the home visit for the most appropriate clinician to attend.
Carry out home visit assessments for those patients fulfilling the NP criteria.
Complete the EMR for each patient visited.
Assess & complete relevant care plans for patients in residential & nursing homes & private homes, where appropriate & within remit.
