Job description
The Post holder will:

- Be a Registered Nurse and have a professional and clinical knowledge of their continence promotion and management or a passion to develop the skills and knowledge to provide excellence in continence care.
- To promote continence or manage incontinence, with clients who are registered with a GP in Staffordshire. Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community clinics, residential homes and clients own home.
- Be skilled in the assessment for continence problems, care and support needs and in the development, implementation and evaluation of treatment plans to improve clients to promote continence.
- Be skilled in supporting clients who require containment management, to prescribe the appropriate appliances with continence pads being the last resort.
- Develop supervisory responsibilities as part of you role for Students and HCA staff.

WORKING RELATIONSHIPS
The post holder to work closely a range of care/support teams/ agencies issues across the Staffordshire, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served. The nature of the service will be determined by local service specifications.
INTERNAL: The Specialist Pelvic Health team, all MPFT clinical teams and corporate teams.
EXTERNAL: GP Practices, Practice Nurses, Residential Care Homes, Domiciliary Care Agencies , the provider of home delivery for containment products,Childrenand adultssafeguarding teams,local government services, Voluntary and Third sector service provision. This list is not exhaustive.
