We are looking to recruit two forward thinking and enthusiastic Care Coordinators to support the three primary care networks and its practices in expanding services available to Warrington patients.
Working for the PCN, the Covid-19 vaccination service and accross GP Practices in:
Central East PCN Greenbank Surgery, Fairfield Surgery, Manchester Road Surgery, Cockhedge Surgery, Holes Lane Medical Centre.
South PCN Stockton Heath Medical Centre, Latchford Medical Centre, Stretton Medical Centre, Brookfield Surgery and Lakeside Surgery
This role will focus on:
Supporting the PCN to achieve goals within the Investment and Impact Fund (IIF) and the PCN Directed Enhanced Service (DES)
Supporting the network with various projects, such as Covid Vaccinations
Managing a caseload of patients
Providing assistance to the PCN Clinical Director
We are looking for three Network Coordinators, each will be assigned to administratively support one of our three PCNs as well as assisting the Clinical Director and PCN Strategic Manager with other projects which may arise, to ensure the PCN delivers its key performance indicators of the Network Contract DES
Requirements of the Role:
The ideal candidate will have or be willing to work towards appropriate training as set out by the Personalised Care Institute.
Central East Warrington PCN provides services to 28,411 patients across five GP Practices
South Warrington PCN provides services to 51,763 patients, across five GP practices.
Across all our PCNs we have embedded new roles under the Additional Role Reimbursement Scheme (ARRS) to support and broaden our primary care team which allows us to provide better care for our patients. These roles include:
* Paramedics
* Mental Health Practitioners
* Social Prescribing Link Workers
* Health and Wellbeing Coaches
* First Contact Physiotherapists
* Care Coordinators
The PCN is headed by our committed Clinical Directors, Strategic Manager, PCN Support Team, Practice Managers and GP members.
In addition to the PCNs our Practices are also part of Quay Healthcare, a non-profit company owned by the GP Practices in Warrington. Quay Healthcare provide business and practical support to the PCNs and deliver additional healthcare services to our patients. The PCNs contract GP provider services through Quay Primary Healthcare CIC who are responsible for providing a number of services on behalf of the PCN; one of these services includes the management of our PCN ARRS staff, therefore this means that you will work alongside members of the PCN and Practice staff on a daily basis, with Quay Healthcare as the lead employer.
Job Structure:
With the creation of the 2020/21 Network Direct Enhanced Service (DES) and the introduction of the NHS Additional Role Reimbursement scheme for new primary care roles, an opportunity has arisen for Primary Care Networks (PCNs) to bring in roles that support GP practices, Networks and elements of the Network DES and this role looks to support this work administratively, linking the PCN and practices with the wider health care community, including but not limited to MDTs, social care, community care, pharmacy teams and care homes.

Network:
Whilst working within the PCN team duties will include:
Providing personal assistance to the Clinical Director
Effective and efficient electronic diary/inbox management (Outlook)
*Handling various telephone enquiries
Provide administrative support to a wide range of meetings including the timely preparation and circulation of agenda packs in readiness for the meetings
Producing clear and accurate documents
Timely preparation and distribution of accurate and relevant minutes and action logs and the follow through actions
Exercise judgement and discretion in the day-to-day handling, monitoring and filtering of appointments and enquiries
Make and receive phone calls, using initiative and judgement when handling enquiries
Take action and provide feedback as appropriate, ensuring that messages are relayed and action is taken
Provide admin duties for the PCNs set by the Clinical Director and Strategic Manager
Build and maintain strong influential relationships with internal and external stakeholders
Run clinical searches to support the networks IIF requirements and further data needs
Support network communications to PCN staff, practices and patients
Support PCN projects to deliver the DES
Organising PCN patient and staff engagement events
