The team act as a gatekeeper to admissions so the post holder will be able to consider referral information from other services and be able to contribute to decision making regarding the most appropriate treatment option. The post holder will also work in our triage hub, responsible for accepting and triaging urgent referrals from a variety of sources, for those experiencing a mental health crisis. You will be expected to maintain a high standard of electronic patient records.
You will work collaboratively with colleagues in the team and across the community mental health service. There will be an expectation that they will work flexibly across the service which covers a 24-hour period, 7 days a week. We are looking for someone who is professional in how they communicate, work with care and compassion, are competent, committed, courageous, reliable, trustworthy and dependable in undertaking allocated tasks and responsibilities. The service has seen many positive developments in a short period, which we continue to play an important role in- supporting individuals in a Step- up House as an alternative to hospital, and having close working links with the Mindline phone and Crisis Safe Space services in the county.
