Job Title: Community Mental Health Nurse - Band 6
Trust Location: Surrey & Borders Partnership NHS Foundation Trus
Pay Rate: Days £36.74, Nights and Saturdays £40.00, Sundays / Bank Holidays £50.00(per hour)
Working hour:· Monday - Friday 9am - 5 pm full time
Shift Pattern: 37.5 hours

What you’ll be responsible for:
- Be accountable and responsible for a defined caseload that reflects the priorities of the local population and carry responsibility for the assessment of care needs, under the supervision of the Clinical Lead Nurse.
- To ensure effective daily management of referrals, including appropriate triage and assessment within specified timescales across the service.
- To ensure appropriate response to emergency referrals, supporting referrals within the acute care pathway and liaising with services involved including HTT, inpatient services and MBU.
- Participate in the multi-disciplinary assessment of women with mental health problems living in the community, by providing a community perinatal mental health nursing perspective and expertise.
- To be aware of the potential for vulnerability to abuse amongst the service users/families, and use of the local policy to inform appropriate personnel in line with safeguarding vulnerable adults and children and SABP Policies.
- Ensure that carer’s support is offered to carers of women that use our service.
- Participate in the provision of community perinatal mental health nursing advice and consultation to colleagues, community groups and organisations.
- Ensure detailed and appropriate records are maintained to demonstrate that contact with people who use services, and their carers’, is meaningful and effective.
- To adhere to the requirements of the NMC Code of Conduct at all times.
- To lead the clinical management of a care package to an individual who uses our services.
- To co-ordinate and monitor the delivery of person centered healthcare to a designated caseload, using complex clinical decision making where appropriate.
- To work as a member of the MDT in assessing and planning care and responding to changes in the health of the women accessing the perinatal service.
- To act in a manner to respect the customs, individuality, values, sexuality, and spiritual beliefs, actively supporting the individual to fulfil these.
- To create, develop and maintain professional supportive relationships as well as joint working arrangements with colleagues in the Perinatal service, secondary care mental health services, maternity services, children’s services, primary health care, social services, and voluntary services.
- To liaise with relevant midwifery and obstetric service around the physical health needs of the women accessing the perinatal service.
- To participate in the maintenance and monitoring of service standards, collect and collate data/ information effectively for the purpose of audit, research, and service performance.
- To contribute to the maintenance, monitoring and implementation of service standards and policies.
- To promote shared decision making with service user/relatives/carers, as appropriate, involvement in all aspects of their care.
- To undertake risk assessments and management strategies in line with trust and local policies and ensure the delivery of care within same.
- To recognize potential crisis situations and act responsively and responsibly to resolve the situation, seeking assistance if required.
- To identify and address health education issues that cause risks relevant to service user group.
- Participate in the education of pre and post registration nursing, social work, psychology, medical and occupational therapy students.
- Supervise junior nurses and other non-qualified staff in the service/team.
The role will involve, but not be limited to you performing the above.
You’ll have the following qualification/experience:
- Band 6 – 2 year’s post qualification experience.
- Suitable for someone who is passionate about ensuring a high-quality holistic approach to the care and support provided to the women who have been identified as in need of perinatal support to enable them to have a positive experience of pregnancy, childbirth and during the first year of motherhood.
- Driving licence suitable for use in the United Kingdom.
