Job description
To be responsible for the summarising and read-coding of relevant information into the patient’s electronic healthcare records on Emis Web clinical system, adhering to the practice notes summarising policy. In addition, the post-holder will also support the multi-disciplinary team, providing administrative support as required, in line with the needs of the practice. Advance Medical Terminology is required for this role.
The following are the core responsibilities of the medical records summariser. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
a. Reviewing records and accurately producing a summary of the patient’s medical history
b. Accurately read-coding data on Emis Web
c. Reviewing progress of summarising ensuring practice targets are achieved
d. Review all clinical letters ensuring they are processed appropriately, in line with extant protocols
e. File and store records as required
f. Request and return patient notes from the storage facility
g. Photocopy documentation as required
h. Action all incoming emails and mail in a timely manner
i. Scanning of patient related documentation and attaching scanned documents to patient’s healthcare records
j. Input data into the patient’s healthcare records as necessary
k. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately
l. Carry out system searches as requested
m. Maintain a clean, tidy, effective working area at all times
n. Support all clinical staff with general administrative tasks as requested
