An exciting opportunity has become available to work under our Access Services in Adult Social Care for 2 Carers Assessment Workers to support main informal (unpaid) carers and the people they care for. The role will involve face to face visits to people living in the community to maximise rapid identification of risks and eligible support needs. This role is central to the Care Act 2014 and has a focus on providing support which will prevent, reduce or delay the need for long term care and support and maximise the wellbeing of those assessed.

2 posts are available at 37 hrs a week. Opportunity for 4hrs overtime a week to meet service requirements when demand is required, for which you will be paid enhancements.

The post holder will be part of the The Carers Team within Adult Social Care and Health. Your focus will be on assessing the needs of the informal carer and joint working with the Carers Hub to begin them on their support journey towards greater wellbeing and resilience to maintain their caring role. You will provide creative and preventative interventions including Wellbing Grants, daily living equipment and assistive technologies as part of wider social care assessments to support both the informal carer and the person they care for. You will provide practical support and identify appropriate referrals and interventions to promote or maintain independence and well-being. Management of risk and identification of timely interventions which can promote independence is key to this role.

Experience of working for Adult Social Care within the community is essential. An understanding of the needs of carers and a ‘strengths based’ approach to support is fundamental. An appreciation of the role of daily living equipment and technology is important but full training on the Enhanced Assessor role and Telecare interventions will be provided.

Your starting salary will be pro rata if the above position is less than 37 hours or term-time only.
