An exciting opportunity for a Health Care Support Worker (HCSW) to join the Hospital@Home team, helping shape the way in healthcare in the community has arisen. Working in a privileged position supporting acutely ill patients in their own home the post holder will work collaboratively with the MDT and clinical groups within the organisation. The HCSW will support the registered nursing staff in the monitoring and safe escalation of the acutely ill patients in both a hospital and community setting. The post holder will be required to work within all areas of Hospital@Home including Emergency Department, patients own home, same day emergency care and within an outpatient setting. The post holder will be trained in phlebotomy and vascular line access care as well as basic wound management.

The post holder will become knowledgeable in Hospital@Home and have good interpersonal skills, excellent communication skills and be enthusiastic. Furthermore, the post holder will enjoy working within a small but innovative team who pride themselves on having an open and honest working environment. The post holder must have comprehensive PC skills, including Microsoft office, email and databases.

Responsible for own actions, ensuring all patient interactions/observations/feedback are communicated to the appropriate healthcare practitioner.

Undertake agreed clinical and non-clinical tasks, including assisting Hospital@Home registered clinicians with line care and removal, patient education, onboarding of patients and monitoring of vital signs following training in defined practices. Ensure referral to the appropriate healthcare practitioner where appropriate.

Working autonomously within the patients home without direct supervision, (e.g., performing venepuncture at a patients home).

Demonstrate technical and/or practical skills to ensure optimum care where agreed e.g. cannulation/venepuncture, ECG, aseptic technique, line removal, performing vital signs.

Organise own daily workload and work regularly without direct supervision e.g. use of and cleaning of identified medical and non-medical equipment (trolleys, ultrasound equipment) and data collection.

Assist in the maintaining of consumables stock levels and ensure that orders are placed in a timely fashion.

Assist with patient care associated with the Hospital@Home service and communicate effectively and empathetically with patients prior to, during and after any procedure or treatmen

Work with team members to deliver education and training in device care and maintenance and patient portal technology.

Support less experienced members of staff in their orientation to the team and role

Utilise IT systems to document accurate and timely patient data and work with the team in data collection and audit of Hospital@Home.

Support the team in expected and unexpected clinical events requiring high but unpredictable levels of physical effort according to patient dependency/clinical need

Maintain a clean, safe environment ensuring adherence to Trust standards of cleanliness, hygiene and infection control at all times.

Promote and monitor adherence to Health and Safety and Trust policy designed to protect healthcare staff and service users from known hazards.
