About the Role
Cornwall Mind is committed to providing opportunities for recovery from mental ill-health and maintaining wellbeing for the People of Cornwall.
Cornwall Mind has been commissioned by the NHS to deliver support for high frequency users of ambulance services, emergency departments and other NHS resources in order to reduce pressures on the NHS in Cornwall. Through the High Intensity User Service (HIU), caseworkers will work one-to-one with frequent users of NHS services to help them access alternative support.
Our High Intensity Service co-ordinators listen to the needs of the service users with a friendly approach, supporting them into finding solutions to their issues, aiming to reconnect them back to a better life, to their families, to their communities and often helping them to access the appropriate services in a less chaotic style.
Key Objectives:
- To promote the HIU service to a wide range of stakeholders and interested parties across the health and care system and beyond.
- To develop robust and active links with SWAST (South Western Ambulance Service Trust) 999 Frequent Caller Team, 111 Out of Hours, ED (Emergency Department) and unscheduled care services with whom you will be working in partnership to engage and support service users.
- To work closely with Social Care, GPs, Mental Health Teams, communities and other key health professionals to facilitate optimal joint working on safe and effective care and support for people with complex needs.
- To identify and feedback opportunities for linkages to other relevant initiatives and resourcing opportunities to NHS Kernow, Volunteer Cornwall and other partners and stakeholders.
- To be able to build a rapport with individuals on a 1-2-1 basis, fostering a relationship of trust and mutual respect whilst maintaining agreed professional boundaries.
- To work with clients and staff to ease discharge from hospital.
