Job description
Assist members of the multidisciplinary team in the assessment, planning, implementation and evaluation of care for patients. Participate in the review of patients care-plans as appropriate in ward rounds, meetings and case conferences, in line with policies and procedures. Report and record any changes in patients physical or mental condition to the nurse in charge. Liaise with other professionals and agencies as required.
To escort patients both internally and externally to the hospital, as required. To endeavour to develop sound professional relationships with patients; to promote active listening and communicate their needs to members of the multidisciplinary team. To ensure that confidentiality is maintained at all times in accordance with the company policies, the Data Protection Act 1998 and the Health Records Act 1990.
