Job description
City of York Council is seeking to appoint a number of Health Trainers to our Public Health Team, on either a full or part time basis.
Regularly voted one of the UK's best places to live and visit, rich in culture and heritage, York is a vibrant modern city still making history.
The capital of Yorkshire, it is visited by 7 million people every year and recognised on a regional, national and international stage. Residents enjoy a high quality of life - there is a real pride in York. People are generous with their time and help one another, particularly the more vulnerable people in our local community. However, there are some significant health inequalities in the city, which we are working closely with partners to improve.
These posts offer an exciting and challenging opportunity for a self-motivated and enthusiastic individual to join our established and lively Public Health Team.
The Health Trainer service is responsible for delivering the city’s stop smoking service and one to one lifestyle advice. The service works with individuals to tackle inequalities in health and support people to live healthy and independent lives by targeting resources on individuals and groups in greatest need.
These roles will lead on the delivery of a new partnership for the team, working more closely with the five Primary Care Networks in the city to embed health trainers in GP practices. They will therefore have dedicated time every week based within York based GP practices, supporting patients who hit certain health and wellbeing criteria in a proactive manner to access onward support, which may include smoking, weight management and diabetes prevention services. This is an exciting opportunity for our team to reach new groups of residents in the city and develop trusted relationships with clinical colleagues.
We would like to hear from you if you are experienced in;
Working in a clinic / NHS based environment
- Smoking cessation
- Health Promotion
- Increasing physical activity
