Health Practitioner for Smoking Cessation Band 5
We are recruiting to a new Smoking Cessation Service, which will be pivotal in helping our patients give up smoking and/or the use of other tobacco containing products through evidence-based interventions. The NHS Long term Plan commits to support every person admitted to hospital who smokes by offering tobacco dependence treatment. This includes expectant mothers throughout antenatal care. Smoking is one of the leading causes of preventable illness and death, and there is evidence from Ottawa and Manchester to suggest that implementing a hospital-based tobacco dependence team can contribute to significantly improved health outcomes for patients and communities. These include reduced morbidity and mortality from smoking and tobacco dependence, improved medical and surgical outcomes, reduced hospitalisations and readmission rates, and addressing/reducing the health inequalities associated with higher rates of smoking in more deprived populations groups and areas.

- Provide high quality smoking cessation behavioural support and pharmacological advice in accordance with evidenced based practice to those who want to give up smoking or use of other tobacco products
- Ensure patients have equitable access to the service, providing individualised care.
- Ensure patients requiring tobacco dependence support are seen in a timely manner, and patients are fully informed of follow up care/appointments.
- Maintain good relationships with patients requiring tobacco dependence support, respond promptly to requests and ensure a high degree of confidentially, complying with the Trust’s values.
- Pro-actively raise awareness of the support that is available to patients through an on-going communications plan
- Work collaboratively with clinical teams, public health, and community smoking cessation services.
- Liaise with the pharmacy team about access to pharmacological support such as nicotine replacement therapy provision for patients
- Maintain accuracy and completeness of all electronic records.
- Keep up to date with evidence-based practices, techniques, and developments.
- Plan and organise a broad range of complex activities relating to the smoking cessation offer, and adjust as required.
- Assist in producing reports and analysis of data that contribute towards improving service delivery
- Responsible for personal communication across a range of agencies within Croydon Health Services and within the wider professional network.
Our main hospital site, Croydon University Hospital is one of the busiest in London. Our experienced district nursing teams, Allied Health Professionals and community matrons look after people of all ages across Croydon and our Children’s Hospital at Home cares for children with long-term conditions without them having to come to hospital.
