Hospital at Home is an exciting service developed by the Thames Valley Provider Collaborative. The Hospital at Home team cares for young people with eating disorders who meet the threshold for inpatient care.

In some cases, admission will be prevented and in others, young people will be stepped down to this model following a short inpatient admission. Hospital at Home maximizes the use of new technology, provides care close to home, reduces admission, length of stay, waiting times and provides earlier discharge from inpatient care.

Healthcare supports workers are pivotal in building rapport and engaging young people in treatment and recovery. Therefore, you will require excellent communication skills, a compassionate and kind attitude and a commitment to advocating for young people and promoting their wellbeing.

You will work as a valued member of the Hospital at Home team to provide compassionate, person-centered care whilst supporting young people to understand, challenge and overcome their eating disorder

You will undertake a variety of duties (including administration) as directed by the Registered Nurse and work closely and collaboratively with the multi-disciplinary team (MDT).

Hospital at Home is an innovative team who place young people and their families at the heart of service provision. The team work closely together to ensure high standards of care are consistently delivered to all of the young people we care for. Communication is key and there are set times each day for the team to meet for handovers and every team member is encouraged to attend weekly ward rounds for each young person.

As a member of the nursing team the post holder is expected to demonstrate attitudinal competence through a professional, approachable and helpful manner towards patients, their families and carers. The post holder is expected to actively support high standards of care delivery and to promote the wellbeing of patients. Your role will include the following:
  • Assist in delivering a high standard of holistic care to patients in a friendly, respectful, and professional manner.
  • Promote patient dignity and privacy.
  • Assist in the orientation of patients and new staff to the service model
  • Contribute to a patient’s admission process in preparation for clinician assessment
  • Contribute to and interpret care plans accurately and consistently
  • Carry out delegated care tasks including supported meals and skills sessions
  • Assist at mealtimes to ensure patients have an adequate diet and bring any problems to the attention of the Registered Nurse or Shift Co-ordinator
  • Assess and promote good physical health and raise any concerns with the Registered Nurse
  • Observe and report any changes in patient’s conditions to the Registered Nurse or shift Co-ordinator
  • Encourage patients to engage in general communication and more specifically to feel able to talk about concerns, needs and wants and feed back communications to the wider multi-disciplinary team
  • Communicate with relatives and carers in a friendly, helpful, and respectful manner
  • Liaise with other relevant individuals involved with the patient as appropriate and agreed with the patient and the registered nurse
  • Write accurate records of care activities and communications with patients, relatives and carers in patient’s notes
  • Support patients with individual needs and choices
  • Give formal verbal and/or written hand-over of patient care
  • To supply all clinical information within the required timescales on the electronic system RiO
  • Assist the team to identify and manage areas of risk in the home environment to safeguard health and safety for patients and their families
  • Report all accidents and incidents to the Registered Nurse or Shift Co-ordinator
  • Participate in the management of emergencies e.g. cardiac arrest and self harm
  • Participate in the management of incidents of aggression
  • Undertake administrative duties as delegated by senior staff
  • To participate in co-facilitation of the therapeutic programme
Professional and Education:
  • Participate in supervision, appraisal interviews and teaching sessions as identified by supervisor and Unit Manager
  • Attend all mandatory training
  • Regularly review own performance and identify and development needs in conjunction with trained nurse
  • Contribute towards consistent and safe practices by being aware of and adhere to all the relevant Trust policies and procedures
  • Promote confidentiality
  • Carry out responsibilities under the Health and Safety at Work Act 1974
  • Keep-up-to date with team, department and Trust development and changes
