DR BATHLA & PARTNERS HEALTH CARE ASSISTANT (HCA) JOB DESCRIPTION Job Title Health Care Assistant (HCA)Role Purpose To provide HCA services to the practice populationReporting to Practice ManagerLocation Soho Health Centre/Hockley Medical PracticeQualification required NVQ Level 2 in Health and Social CareRole Purpose To provide Health Care Assistant (HCA) services to the practice population(s).Supported by the nursing team within the practice, the Health Care Assistant (HCA) will deliver care within the boundaries of their role and in accordance with specific practice guidelines and protocols.Responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making within the parameters of their qualifications and role requirements.They will focus upon supporting patients to be healthy by assessing their needs, providingHCA care, health education advice, screening activities, implementing care at the surgery, community or other settings, maintaining clinically accurate records and running clinics.They will work collaboratively with the general practice team to meet the needs of patients and support the delivery of policy and procedures, and providing HCA guidance as required.Duties and Responsibilities Clinical Practice The post holder will make professionally decisions, providing safe, evidence-based, cost- effective, individualised patientcare. To include;Health Checks o Blood Pressure, Height, Weight, BMI o NHS Health Checks including referralsHealth and Well-being o Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being o Support patients to adopt health promotion strategies and encourage principles of self-care o Offer holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care o Weight Management & AdviceVenepunctureChronic Disease management o Undertake appropriate investigation, clinical observations and interventions following agreed guidelines for patients with chronic diseases CHD, Diabetes andHypertension, Asthma, CKDEmergency Care o Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.Wound Management o Removal of stiches and clips o Meet the needs of patients presenting for wound care and use dressing formularyMental health, adult and child safeguarding o Recognise patients presenting with mental health needs in accordance with theNational Framework (NSF) for Mental Health. Raise relevant information with the duty GP. o Recognise adult and child safeguarding situations and act appropriately o Learning Disability annual check with clinician/GPVaccination programmes o Provide vaccination and immunisation programmes for Adults following guidelines for flu, pneumovax, shingles, B12Patient Profile Activities o Review patient data to work with the nursing team, to achieve focused patient healthcare o Active participation in achieving QOF and Enhanced Services targets and recall demandsContribution to the implementation of ServicesThe post-holder will;Apply national, local and practice policies, standards and guidanceDiscuss with team members how the policies, standards and guidelines will affect own workParticipate in audit where appropriateDeliver a quality serviceThe post-holder will strive to maintain quality within the Practice, and will;Effectively manage own time, workload and resourcesReflect on own and team activities to identify risks, enhance performance and improve quality of patient careWork within own competenceProduce accurate, specific and complete records of patient consultationDeliver care according to the NSF and the NICE guidelines and evidence based care.Understand and apply statutory procedure and local guidance regarding the identification of vulnerable adults and children, domestic violence, substance abuse and addictive behaviourJob Types: Permanent, Part-time, Full-timePart-time hours: 20-40 per weekSalary: £11.00-£11.50 per hourSchedule:Day shiftMonday to FridayAbility to commute/relocate:Birmingham, B1 3AL: reliably commute or plan to relocate before starting work (required)Education:GCSE or equivalent (preferred)Experience:Care Assistants: 5 years (preferred)Work Location: In person
