Gentleman in Norwich seeking cover PA for 4 days a month @ £11 p/h

I am a friendly gentleman who is seeking a carer who can cover on average 4 days a month. These days will run consecutively, usually from a Friday to the Monday. During this time I require two calls per day, one in the morning for 1.5 hours and one in the evening for 1.5 hours. I am flexible with the exact timings and am happy to discuss this at interview. I do, however like to go to bed early and would prefer someone who is able to do the evening call between the hours of 5pm-7pm.
I am prone to falls and have dizzy spells when I stand. I have difficulty walking and tend to shuffle when I do, as a result when I go shopping my carer takes me out in a wheelchair. I am diabetic and have epilepsy, both of which are controlled by medication and I haven't had an epileptic fit for several years.
I wold like someone who is able to help get me washed and dressed and prepare a cooked breakfast for me in the mornings, something like eggs on toast and a light meal in the evenings. I will need help changing my pads, so someone who doesn't embarrass easily would be ideal. I will also, on occasion, need my prescription collected from the pharmacy.
I am a well-travelled gentleman, my father was in the Air Force when I was growing up and as a result I have lived all over the world from Singapore to Germany and of course the UK. I enjoy knitting and playing chess and I am currently knitting some clothes for my grandchild.
I would ideally prefer a carer who is self-employed. The self-employed rate for this position £13.87 per hour. A self-employed carer will be required to be registered with HMRC for self-assessment and obtain public liability insurance.

Summary terms and conditions:
- Hours and days of work: 3 hours per day for 4 days per month. 1.5 hours in the morning and 1.5 hours in the evening
- The rate of pay for this position is £11 per hour
- You will be reimbursed for any agreed expenses incurred as part of the role
- The annual holiday entitlement for this position will be equivalent to the work I would normally expect to be done during 5.6 consecutive weeks
- You will be required to undertake annual training If this job requires any of the following:
- Manual Handling
- Handling of hazardous chemicals
- Food Handling
Should you choose to work on a self-employed basis the rate of pay will be £13.87 per hour. You will be required to register with HMRC for self-assessment and obtain public liability insurance.
The post is subject to the following:
- A satisfactory Disclosure & Barring Service (DBS) check, the cost of which will be covered by the employer
- Satisfactory references from two referees, one of which must be your current or last employer
