Job description

You will act as a specialist nurse in in the overall assessment and management of patients with complex health needs on discharge from the acute hospital setting.

You will work within clearly defined local and national policies and standards with regards to discharge planning.

You will be responsible for facilitating and providing high quality, patient focused care with responsibility for assessing, developing and implementing specialist discharge planning programs.

You will be responsible for providing specialist advice and support to, and liaising with, other staff & wider partners.

Identification, Assessment, planning, implementing and evaluating care as a practitioner with specialist Discharge Planning Skills. This involves the understanding and completion of specialist discharge planning assessments such as those within the full CHC process, Special Needs Funding, FNC’s, health equipment and the fast track assessment of those patients entering the terminal stages of their life in accordance with local and National guidelines and the application of these assessments against the eligibility criteria’s associated with each assessment type

  • Complete specialist assessments to submit to commissioning panels acting as an advocate for patients.
  • Provide advice and clinical expertise to staff and community colleagues including those from Nursing and Residential homes to expedite discharge and where possible prevent admissions.
  • Provide specialist clinical advice for patients and carers
  • Carry a caseload, and take responsibility for the management of allocated cases ensuring that there is appropriate liaison with relevant members of the MDT to ensure discharges are achieved in a timely manner.
  • Identify the need for and lead case conferences to enable patient choice, and a safe and timely discharge
  • Provide a high level of communication by:
Liaising with all appropriate members of multi-disciplinary team

Communicating very sensitive condition related information to patients and relatives, requiring high levels of empathy and reassurance.
