Job description

An exciting opportunity to join us at Forward Thinking Birmingham in our Urgent Care Service, you will be responsible for co-ordinating both early and complex discharges for our young people of Birmingham in an acute setting minimizing delays and maximizing bed capacity.

We are looking for an individual with a good understanding of acute inpatient discharge who will be committed to driving the discharge process, identifying and anticipating any issues that may arise, working closely with the MDT and developing effective relationships with both internal and external agencies.
