Seisdon Primary Care Network are looking for a part time dynamic and enthusiastic Digital Transformation Lead to work alongside our current lead and join our Primary Care Network.
The Digital Transformation Lead will work with clinical and operational teams within Perton, Codsall, Bilbrook, Featherstone, Wombourne and Claverley practices and Stafford and Stoke ICB Primary Care teams.
As Digital and Transformation lead you would be expected to support increased access to care for patients, by supporting the adoption and/or optimisation of new technology and other initiatives to improve the care offer and enabling practice staff to work more effectively to support the sustainability of general practice services. The core objective of the PCN Digital & Transformation Lead role is to support the delivery of ongoing improvement to administrative and clinical services. The role will require experience in business/service improvement with expertise in digital and transformational change and a thorough understanding of general practice systems and processes is essential.
- Hours: 15 to 22.5 hours per week
- Salary: Up to £51,400 pro rata dependent on experience

Job Responsibilities:
Work closely with management, GP partners, and staff to understand current service models and admin processes, identify challenges, agree priorities for improvement, and support delivery.
Work closely with the ICB Primary Care/ Digital teams to ensure local transformation plans and activity are aligned with the ICB priorities e.g. improving access, PCN Development, the ICB Digital Strategy.
We are a small PCN and already work extremely effectively as one team across all sites. As part of our maturity as a PCN, we now need to ensure that processes and policies are fully aligned, standardised, and maintained in the longer term. We already work effectively across the PCN, we now need to fine-tune with the post holder being the enabler to support population health management which is becoming an increasingly important part of the transformation of primary care, along with supporting PCN DES requirements.

Develop and manage an overall transformation plan for the PCN, including identifying interdependencies, managing risks, considering potential impacts on the wider organisation, and determining resource requirements.

Seisdon PCN currently has a strongly embedded team of ARRS staff: Pharmacists, Social Prescribers, Adult Mental Health Practitioners, First Contact Practitioner plus we are also recruiting for further roles to support the future plans of the network. We expect the post holder to work very closely with the ARRS team to help develop and promote their services to both patients and staff.

Qualifications
- Professional qualification or experience in IM&T to demonstrate subject matter expertise
Experience
- Must have active experience working in Primary care using Emis Web on a daily basis & provide evidence/ability to be able to run searches and data set runs using Emis Web.
- Experience in Digital/IM&T project management delivery through the entire project lifecycle from initiation to delivery;
- Good understanding of the Digital agenda within the NHS and Social Care / Care
- Ability to work on own initiative and organize workload, allocating work as necessary;
- Used to working in a busy environment;
- Adaptability, flexibility and ability to cope with uncertainty and change.
- Ability to deliver a vision and a willingness and drive to adapt to NHS targets and business planning, whilst keeping abreast of legislative and contractual changes;
- Self-motivation and forward-thinking approach;
- Excellent organisational, management and planning skills, IT skills and communication skill
- Ability to implement and manage change effectively.
- Experience of using Ardens would be an advantage
