The team
Liaison Care is at the heart of supporting the NHS standardise and improve the end-to-end delivery of Continuing Healthcare (CHC). This ensures that Integrated Care Boards (ICBs) can guarantee everyone has fair access to CHC and can ensure that patients and families have better experiences and outcomes, whilst helping the NHS make better use of its resources in this area.
Liaison Care has developed a range of products and services to support ICBs transform and manage the challenges of delivering CHC. These include transformation reviews, expert advice and software to help accurately predict personal health budgets.
We are currently working with some prestigious NHS Organisations to help them to reduce their backlog of cases and reviews.
Main responsibilities
As Care Delivery Lead you will take responsibility for delivering specified work packages for NHS Continuing Healthcare, and associated projects.
The position requires someone to lead and manage a diverse team of staff including social care, nursing staff and Associate Practitioners.
You will oversee the quality assurance in line with the national framework and client service specifications, including Reviews, and Decision Support Tools.
You will also be required to provide leadership for quality assurance service delivery and improvement.
Experience and qualifications required
Essential experience:
- Experience in managing staff and Human Resources issues such as appraisals, CPD, resolving conflict, absence / sickness managing and identifying training needs of staff
- Ability to identify risks, anticipate issues and create solutions and to resolve problems in relation to project or service delivery
- In-depth knowledge of NHS Continuing health care policy and procedures
- Evidence of continuing professional development (CPD)
- Ability to oversee a work package to ensure excellence in practice in reviews, CHC and FNC
- Essential Qualifications:
- Registered Nurse, Allied Health professional or Social Work qualification
- Degree level qualification or equivalent knowledge training and experience
