We are currently looking for a Registered General Nurse or Mental Health Nurse to join our team. This is an opportunity to be part of a team of skilled clinical and administrative staff who undertake the assessment of individuals for NHS funding for their care.
Our Nurses undertake full assessments and reviews of eligibility using the NHS Framework for Continuing Healthcare. In South Devon we are responsible for case managing those individuals who are in receipt of full Continuing Healthcare funding for their care.
We work alongside multi-disciplinary teams to ensure individuals needs are reviewed and met. We assess, plan and commission care. The role can often involve responding to crises in care provision and problem solving with the support from the CHC team and wider MDT.
This role is both interesting and challenging with plenty of opportunity to develop a range of skills and to work alongside others with a shared goal of providing the right care for those in our community with complex health needs.
- Excellent holistic assessment skills.
- Sound clinical knowledge
- Proficient in assessing and documenting all aspects of the mental and physical health of individuals.
- Work autonomously
- Prepared to travel to Nursing Homes, Residential Homes and individuals own homes in the community.
- Awareness of and adherence to infection prevention and control procedures.
- Coordinator of the Continuing Healthcare assessment process.
- Be mindful of the rights of individuals to be screened for assessment of eligibility and their right to representation and advocacy.
- Proficient in managing situations where there is conflict or disagreement.
- Be involved in the appeal process
- Build on existing analytical and assessment skills in order to plan and co-ordinate safe and effective care in collaboration with the individual and with other health and social care professionals.
- Make referrals to allied health professionals and to Safeguarding and Quality teams.
- Spend long periods of time sitting and using a computer to type up assessments. Candidates therefore should be proficient in information technology and governance.
- Work within a multi-disciplinary team of clinicians and administrative staff. Having excellent communication and interpersonal skills.
- Work effectively in a team, respecting individuality and the contribution of others.
We have an experienced team of Nurses from various backgrounds, we are able to offer excellent peer support and advice. This means that when there are challenges and unknown territory, there is always support on hand or the ability to effectively signpost in order to uphold the rights of individuals to thorough and robust assessments.
The role involves consideration of financial resources and a recognition of not only safe and effective care but value for money so that we can provide the most cost effective solutions using the publicly funded NHS. This may not always be familiar to new recruits to the team, however we have our own finance team who provide essential support advice which every member of the nursing team benefit from on a regular basis.
