Job description
We are looking for a motivated and experienced assessment nurse who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments for CHC and FNC eligibility.
The successful candidate will need to be resilient, have attention to detail and a positive approach. You will have a good understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, excellent communication and problem-solving skills and have a proven ability to produce high quality documentation whilst working to deadlines and targets.

We are currently looking for an adaptable and enthusiastic Assessment Nurse to join our ever-expanding Nurse Team in the Continuing Healthcare Department.
The role
The successful candidates will provide specialist nursing assessment, review and case management of adults and older people residing in some acute settings, care homes with nursing, residential care settings and individuals within their own homes, in line with the National Framework for NHS Continuing Healthcare and the determination of the Funded Nursing Contribution (FNC).
To work closely with local partners, to include health, social care and independent providers in the planning, commissioning of care, management and review of patient care needs. Provide clinical advice in relation to NHS Continuing Care and NHS Funded Nursing Care (FNC).
To provide assurance around quality of commissioned care packages, responding to and reporting safeguarding concerns in accordance with agreed interagency reporting procedures.

- To deliver specialist nursing assessment for adults and older people currently receiving long-term nursing care, being considered for long-term nursing care or being considered for continuing healthcare funding.
- To prepare appropriate documentation to enable the level of NHS funding to be determined within the parameters of the National Framework for Continuing Health Care and Funded Nursing Care.
- To seek authorisation of eligibility recommendations via IPA Team Leader.
- To procure appropriate packages of care for service users eligible for NHS funding.
- To be responsible for case management and review of clients eligible for NHS Funded Care.
- Act as duty clinician on a rota basis.
- To process urgent fast track referrals and, where appropriate, procure packages of care in a timely manner for those service users who are receiving or require end of life care.
- To provide assurance regarding quality of care of individual packages by undertaking reviews as agreed.
- To report quality/safeguarding concerns through agreed reporting routes in a timely way.
- To undertake urgent reviews of clients where quality and safety concerns have been identified.
- To actively promote communication and partnership working between the statutory, independent and voluntary sectors.
- To communicate relevant information and to provide advice and support on NHS funded care/continuing health care to service users and carers.
- To liaise with other health professionals on clinical matters as part of the assessment process.
- To maintain professional confidentiality in all matters relating to patient care.
- Promoting of the use of and support the pathway delivery for Personal Health Budgets for individuals as a means of their having control over their care and achieving best possible outcomes whilst being cost effective and value for money.
