Job description
Leicestershire and Rutland are rolling out ‘The 3 Conversation Approach’ to help people who are stepping down from secondary mental health services and people where engagement with secondary mental health services has not been successful.
The 3 Conversation Approach is an evidence and strength based approach developed by Partners4Change which focuses on coming alongside people to identify what a good life means and the connections that can be made to best enable this to happen. The Approach moves away from eligibility and formal assessments to relational based conversations which are led and shaped by the person.
As part of the approach, we are hiring four Community Reablement Workers to deliver a community-based enablement and inclusion service, which supports people to learn or relearn skills, enables individuals to become as independent as possible, reduces social isolation, and encourages people to be part of their community.
If you have relevant health or social care qualifications or experience, can work collaboratively and are confident giving guidance and support, we'd love to hear from you.
