Adult Social Care Reviewing Team - full time / permanent

Exciting opportunities have arisen in the Adult Social Care Service within Sefton Council. The Reviewing team are seeking applicants with backgrounds in community work and experience of working with service users with learning disabilities and EMI would be a distinct advantage.
As a community assessor, you will contribute to the operational delivery of an effective Adult Social Care service as part of a team responsible for promoting the welfare of and continuously improving outcomes for individuals.
You will manage a defined case load delivering high quality reassessments and support plan reviews that address the needs of vulnerable people through the commissioning of individual packages of care, whilst managing the reputation of the Council.
