Job description

An exciting and rare opportunity has arisen for and experienced and motivated children's and/or learning disability nurse to lead and transform the Special School Nursing Team in Ealing. The team is part of the Children's Specialist Community Nursing Service in Ealing and the post holder will be expected to closely work with the Clinical Team Leads for the Children's Community Nursing Team and Concare Team, as well as the Clinical Nurse Specialists within the service.

The post holder will be leading and supporting a team of Qualified Nurses and skilled Health Care Assistants (HCA's) delivering support/intervention to children with complex medical and health care needs within one of the six special schools within the London Borough of Ealing. The post holder will be responsible for developing and standardising training resources, standard operation procedures, provide clinical oversight and supervision to staff. The post holder will have close working relationships with the other Clinical Team Leads and also establish professional working relationships with Ealing's' Allied Health Professionals, Community Paediatric Consultants and peers across Special School Nursing Services within the Trust and within Universal Services. The service links closely with the local authority social care team (Children with Disabilities Team) and SENAS team.

JOB SUMMARY
  • To lead and manage the delivery of care and/or treatment to children and young people known to the Special School Nursing Team. To be aware of operational procedures for the children’s community nursing and continuing health care teams and clinical nurse specialists within the service, in line with the Trust’s strategic direction, ensuring the delivery of high value, patient centred, responsive and efficient patient care, as operational cross cover for the two other Clinical Team Leads will be required for absence.
  • To work effectively with the Clinical Service Lead and Clinical Service Leads for other teams, such as universal services, physio, SLT, OT etc. and the Heads of Special Schools and other stakeholders, ensuring effective communication and partnership working, as and when required or asked to represent the Clinical Service Lead.
  • To take responsibility on behalf of the Clinical Service Lead for the delivery of Special School Nursing Services, ensuring an integrated approach with the wider specialist services and other agencies, focused on meeting the needs of a designated caseload.
  • To ensure services are delivered in a highly professional and clinically safe manner, in conjunction with the business support team and in accordance with local and national policies and procedures, including compliance with Care Quality Commission standards. The post holder will retain at least 0.4 WTE clinical time.
  • To provide management and leadership to staff within areas of responsibility, ensuring implementation of and compliance with policies and procedures.
  • To take responsibility for ensuring effective capacity and demand management across the Special School Nursing team, in order to meet agreed activity levels, in line with the wider children’s services performance plan.
  • To be responsible for ensuring outcome measures are developed and implemented within areas of service responsibility.
  • To be aware of the resources / budget of the team and support Clinical Service Lead with management of resources for the service.
KEY DUTIES/RESPONSIBILITIES

Operational Management
  • Take responsibility on behalf of the Clinical Service Lead for the operational day to day management and continuity of the Special School Nursing team.
  • Provide leadership to and be responsible for team members in the day to day running of the team, promoting multidisciplinary working and ensuring organisational and divisional barriers do not prevent the development and functioning of integrated care pathways and high-quality patient care.
  • Undertake regular audit of adherence to associated local and national policies, guidance (e.g. NICE) and standards of care.
  • Contribute to the achievement of standards within the areas of responsibility, working effectively with other Clinical Team Leads and clinical staff to ensure agreed standards are maintained.
  • Prepare and deliver team reviews as agreed, reporting on the performance of the team within area of responsibility.
  • Support service and Clinical Service Lead to ensure effective leadership, supervision and management of resources.
  • Investigate and respond to complaints and incidents/serious incidents within a specified timeframe, analysing and monitoring data collected and collated from the team, to ensure that action is taken to correct and prevent recurrence and report to the Clinical Service Lead on a regular interval.
  • Ensure escalation processes are understood and embedded within the Special School Nursing team.
  • Take responsibility for performance management and achievement in the team, including management of effective clinical governance and enhancing patient safety, and report to the Clinical Service Lead when there are concerns regarding performance and/or standards.
Strategic Management
  • Work as part of the wider Specialist Children’s Community Nursing Service and contribute to the development of plans, which will support the strategic direction of the Trust and wider local Health, Education and Social Care economy; leading on implementation of appropriate service specific plans for the team.
  • Be aware of the impact of any new national initiatives and legislation within the locality, ensuring successful action planning, implementation and compliance.
  • Support the Clinical Service Lead with the development of written policies, procedures and protocols in line with the agreed development of the services within area of responsibility.
  • Be responsible for ensuring compliance with all Trust policies and procedures, including the development of service area procedures and policies as required.
Service Development
  • Take responsibility for the identification and delivery of service improvement and transformation opportunities, working with the Clinical Service Lead to achieve standards of best practice and to meet the challenges of access targets, including the management of specified project.
  • Identify team/service priorities and develop action plans for service improvement within the locality; actively engaging with service users, clinicians and other internal and external stakeholders as required and agreed with the Clinical Service Lead.
People Management
  • Provide managerial leadership to staff within areas of service responsibility, ensuring implementation of and compliance with policies and procedures.
  • To provide clinical supervision for direct line reports and ensure supervision structures are in place
  • Ensure that all direct reports are regularly appraised (within 12 months of last appraisal), development plans are completed and objectives agreed.
  • Support the Clinical Service Lead with all aspects of Human Resource management, supported by the HR Lead and associated HR services, including recruitment, retention and individual performance management
  • Provide effective leadership to the team within the area of responsibility, ensuring strong clinical leadership is established and supported
  • Support the Clinical Service Lead with the investigation and responding to issues relating to disciplinary, grievance, management of sickness and absence and poor performance, utilising resources available and in liaison/partnership with HR and other appropriate personnel.
Communication
  • Establish effective means of communication within the team, so that all staff are informed and engaged with the service and Trust’s priorities.
  • To participate in CSCNS CIGs and Clinical Team Leads meetings, ensuring that the key priorities and service issues of each are of responsibility are appropriately represented.
  • Produce papers and reports as required or as directed by the Clinical Service Lead.
  • To act as an ambassador for the Trust at all times when engaging with patients, commissioners, stakeholders, other external organisations and members of the public.
Resource Management
  • Participate in the production of relevant business cases, including the identification of priorities in line with clinical need, national and local targets.
  • Initiate plans to deliver service improvements and development in line with benchmarks and best practice.
Information Management
  • To work with others to develop the information management needs of the team to enable performance to be assessed against all key areas.
  • Ensure all aspects of the team adhere to agreed processes and standards of reporting against performances standards/targets.
  • Support the Clinical Service Lead with monitoring service and other key performance indicators, and contribute to excellence through taking actions that ensure ongoing achievement of Trust objectives.
Client/Customer Care
  • The post holder will be expected to carry out clinical duties as part of their role particularly in ensuring service continuity due to staffing issues etc.
  • Pre-empt complaints through the presence of systems and staff that can resolve issues promptly and locally before they become formal.
  • Respond to complaints and feedback from patients and public on service delivery, involving liaison with patients and relatives as necessary, dealing with difficult issues in a sensitive way and reports on these to the Clinical Service Leads when they arise.
  • Support the Clinical Service Lead ensuring patient and public involvement is integral to the planning and delivery of clinical services.
  • Be responsible for the delivery of effective clinical and corporate governance within the team, ensuring systems are compliant with policy.
  • Ensure that risk assessments are undertaken on an ongoing basis and resulting action plans are implemented, and foster a culture of open reporting and analysis.
General
  • Maintain a broad understanding of the work of children’s nursing services and the Trust as a whole, and actively contribute ideas for the improvement of service provision.
  • Ensure own actions contribute to the maintenance of a quality service provision.
  • To be responsible for the self-development of skills and competencies through participation in training and development activities, and to maintain up to date technical and professional knowledge relevant to the post.
  • Ensure own continued professional development, maintaining clinical and managerial credibility.
  • Participate in Trust’s Performance and Development Review and to undertake any identified training and development related to the post.
  • Undertake statutory and mandatory training as deemed appropriate by the Trust.
  • Develop and maintain effective working relationships with colleagues.
  • Adhere to all Trust policies and procedures.
This role also requires the post holder be mobile, as there are six special schools within the borough, although the main base for this post is at Carmelita House.
