Patient Navigator to provide support and guidance to participants of the Moms2B Program whose goal is to help pregnant women have healthy pregnancies, deliveries and babies living in a diverse low-economic area of Columbus known for one of the highest mortality rates in the state;, assists program participants with navigating through the variety of community and health care services available to them; coordinates Moms2B sites; serves as a liaison to The Ohio Collaborative to Prevent Infant Mortality; uses case finding techniques to identify and recruit pregnant moms into early enrollment for prenatal care; provides enrolled participants educational and intervention counseling, materials, and access to prenatal-postpartum and inter-conception care; provides guidance and access to an allocation of community resources, education, outreach, home visits and referrals; directs enrollment of health and developmental education classes; facilitates and coordinates appointments with agencies and providers to ensure timely delivery of community, academic, career and service referrals; collaborates with participants to develop a follow-up plan to ensure that referred services meet the present needs and to ensure that future needs are addressed; advises individual participants and family needs in navigating health care system; researches and engages with available services and programs external to The Ohio State University (OSU); attends and represents OSU at community meetings; assesses, maintains and coordinates up-to-date resources and materials used in the program; maintains communication with local area agencies and staff to share information, activities and resources; collects, analyzes and compiles research data for inclusion in progress reports, publications and for dissemination at workshops and conferences; ensures activities align and comply with specific protocols in collaboration with Moms2B Program Manager and Director.
Minimum Qualifications
Bachelor's degree or equivalent experience. 0 years of relevant experience required. 0-2 years of relevant experience preferred.
