Duties

This includes a broad spectrum of disease state management under individual pharmacist scopes of practice including, but not limited to; diabetes, hypertension, hyperlipidemia, anticoagulation, heart failure, COPD, pain and geriatric deprescribing. The incumbent is also responsible for providing oversight and expansion into specialty areas such as dermatology, endocrinology, cardiology, rheumatology, mental health, and geriatrics to name a few. The incumbent will also be responsible for maintaining and evaluating scope of practices for direct reports and providing any changes in recommendations of staffs' clinical privileges to the Associate Chief of Pharmacy, Outpatient Services and/or to the Chief of Pharmacy Services. Furthermore, the incumbent will be responsible for evaluating and analyzing data driven metrics and implementing initiatives to ensure compliance with Clinical Pharmacy Practice Office, regulatory bodies, national and local policies, and national and local initiatives.
Monitors pharmacy staff trainings are maintained and documented with acceptable skills & competency and assists with training all incoming pharmacy personnel.
Performs follow-up quality assurance reviews and develops reports and presentation as needed regarding findings and identification of potential medication errors/near misses.
Identifies any deficiencies and develops appropriate action plan for correction including a sustainment plan.
Functions as the oversight of the hospital and pharmacotherapy staff in the management of the patient's drug therapy, monitoring, and desired clinical outcomes.
Functions as the clinical educator/trainer and as a consultative resource on all aspects of drug therapies and issues. These decisions cover all aspects of pharmacotherapy and require specialized knowledge, skills and sound professional judgement.
Coordinates, supervises, and provides the safe and rational drug therapy management and monitoring to the patients within the hospital, home care, mental health, spinal cord injury center, blind rehabilitation center and general medicine / specialty / community based outpatient clinics assigned to program staff.
Identifies and resolves issues within the area by collaborating with appropriate stakeholders if necessary to assist with improving staff efficiency and care to patients.
Develops and implements action plans to improve staff's morale and reduce burnout.
In collaboration with the Associate Chief of Pharmacy, Outpatient Services establishes work schedules and allocates staff as needed to provide adequate Clinical Pharmacy Program coverage. Notifies the Associate Chief of Pharmacy, Outpatient Services and designated pharmacy timekeeper of any changes in time and attendance records of the Clinical Pharmacy Program staff to ensure accurate accountability.
Assesses workload shifts throughout the day, identifying backlogs and makes staffing adjustments as needed to meet patient care needs and organizational goals. Works collaboratively with the Associate Chief, Outpatient Services and the other Pharmacy Service Leadership Supervisors to ensure continuity of patient care.
Develops and establishes policies and procedures defining the clinical/ professional and administrative operational requirements for the assigned Clinical Pharmacy Program.
Monitors and ensures staff compliance with OPPE/FPPE requirements and clinical privileges as required by VA policy.
Ensures assigned administrative projects and reports are submitted in a timely fashion.
Assists in assuring that the Clinical Pharmacy Program is compliant with all regulatory and accrediting body requirements, i.e., Joint Commission for Accreditation, National Patient Safety Goals, FDA, ASHP, etc.
Serves on hospital or departmental committees as requested. (16) Evaluates, designs, implements clinical activities as requested by the facility and/or VACO.
Assists in conducting and reporting internal audits as required. Maintains all pertinent professional and administrative records as necessary. Supervisor ensures subordinate staff if assigned for Medication Use Evaluations (MUEs) and participates in quality assurance activities, their data collection is beyond reproach.
Evaluating and reporting pertinent metrics to stakeholders to assist with the progression of Clinical Pharmacy Specialists.
Assists with and develops business plans to expand clinical services within the outpatient services.
Works closely with other pharmacy managers to understand and communicate how other areas of pharmacy directly impact the incumbent's staff. Conducts process improvement and/or quality assurance projects to assess program performance based on national, VISN and local performance measures.
Develops long range program planning and development.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)

English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).Education.
  • (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
OR
  • (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure.
  • Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
Grade Determinations:

Experience or Education. In addition to the basic requirements, candidates must meet one of the following:
  • 1) One year of experience equivalent to the next lower grade level, GS12*
OR
  • 2) Completion of an ACPE-accredited Pharm.D. program.
AND

Able to perform Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.

a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

GS13
Responsible for the professional and administrative management of an assigned area in pharmacy service, to include budgetary execution. Such individuals have responsibilities for supervising multiple pharmacists and technicians. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Develops performance standards, assures that the program area is compliant with all regulatory and accrediting body requirements, designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities. Develops policies and procedures relative to their assigned area.

Pharmacists assigned to this position must demonstrate the following KSAs:
a. Ability to effectively supervise subordinate staff.
b. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
c. Expert understanding of regulatory and quality standards pertaining to pharmacy.
d. Skill in managing people or programs.


Preferred Experience:
  • PGY2 Health Systems Pharmacy Administration and leadership or equivalent Ambulatory Care Supervisory experience preferred.
  • Maintains working knowledge of all areas and staffs as appropriate and assigned to ensure competency.
