Job Title

Completion of Independent Prescribing course is desirable. If IP course completed hourly rate is £30.
If IP course is not complete hourly rate is £25 with a view to self-funding and completing the IP course.

Job Summary
A Prescribing Pharmacist is considered to be of a similar level to the Advanced Nurse Practitioner role. You will work as a part of a team of doctors, nurses, nursing associates etc. to deliver primary care medicine in a GP Surgery setting. Your direct supervisor will be GPs in leadership roles with experience in supervision. You will be expected to perform a variety of roles, amongst them:
- Patient contact via telephone or face to face encounters – where you will use the skills acquired as a prescriber in: history taking, examination, differential diagnosis and the formulation of a management plan.
- Document workflow: coding, altering medication regimes as per hospital recommendations etc.
- Management of chronic disease registers
- Medicines management (e.g., acute and repeat prescription requests, simple medication reviews, structured medication reviews, audit work set by the medicines management team at the CCG/ICS)
- Supervision, training, and advice for colleagues where appropriate.
- Participation in clinical governance activities – including clinical audit etc.
You must familiarise yourself with the EMIS Web clinical operating system and stay up to date with necessary Bluestream Training modules. You must also partake in yearly appraisal and staying up to date with clinical knowledge.

Induction Training
On arrival at the practice all personnel are to complete a practice induction programme; this is managed by the Deputy Practice Manager.
Learning and Development
The effective use of training and development is fundamental in ensuring that all staff are equipped with the appropriate skills, knowledge, attitude and competences to perform their role. All staff will be required to partake and complete mandatory training as directed by the training coordinator, as well as participating in the practice training programme. Staff will also be permitted (subject to approval) to undertake external training courses which will enhance their knowledge and skills, progress their career and ultimately, enable them to improve processes and service delivery.
Collaborative Working
All staff are to recognise the significance of collaborative working. Teamwork is essential in multidisciplinary environments. Effective communication is essential and all staff must ensure they communicate in a manner which enables the sharing of information in an appropriate manner.
Service Delivery
Staff must adhere to the information contained with practice policies and regional directives, ensuring protocols are adhered to at all times. Staff will be given detailed information during the induction process regarding policy and procedure.
Security
The security of the practice is the responsibility of all personnel. Staff must ensure they remain vigilant at all times and report any suspicious activity immediately to their line manager. Under no circumstances are staff to share the codes for the door locks to anyone and are to ensure that restricted areas remain effectively secured.
Professional Conduct
Staff are required to dress appropriately for their role. Administrative staff will be provided with a uniform whilst clinical staff must dress in accordance with their role.
Primary Responsibilities
The following are the core responsibilities of the clinical pharmacist. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- Practice as an independent prescriber working within scope of practice
- Carry out face-to-face and telephone consultations with patients with minor illnesses
- To provide medication review services with patients in the practice and during domiciliary visits to the local nursing home
- To provide advice and support for patients with self-limiting conditions
- Encourage cost-effective prescribing within the practice
- Review all safety alerts ensuring the required actions are taken
- To reconcile medicines following patients discharge from hospital or clinic appointments
- Review medications for newly registered patients
- Implement and embed a robust repeat prescribing system
- To provide subject matter expertise on medication monitoring, implementing and embedding a system
- Carry out targeted medication reviews for high-risk patients
- To reauthorise medication for repeat prescribing within scope of practice
- Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required
- Review and implement safe prescribing systems for vulnerable patients, improving compliance
- Support clinicians with the management of patients suffering from drug and alcohol dependencies
- Actively signpost patients to the correct healthcare professional
- Assist with QOF targets
- Administer influenza and travel vaccines
Secondary Responsibilities
In addition to the primary responsibilities, the clinical pharmacist may be requested to:
- Partake in audit as directed by the audit lead
- Provide education on medicines for the clinical team
- Review the latest guidance ensuring the practice conforms to NICE, CQC etc.
- Act as the practice point of contact for all medicines related matters, establishing positive working relationships
- Produce pharmacy / prescribing newsletters or bulletins on a quarterly basis
- To line manage the dispensary staff in accordance with current practice policy
