To ensure all CKD patients starting renal replacement therapy have had all the appropriate education to allow them to have an informed choice of their options for renal replacement therapy Mange the CKD team in the absence of the manager Undertake the co-ordination and the management of CKD patients with renal anaemia Implement individual education programmes for patients and carers entering the pathway Assist with continuing development of health promotion for CKD patients Undertake the continuing development of the End of Life care (EOL) for CKD patients Constantly updating one own knowledge , recognising the importance of a critical approach to research and developments in nursing Be actively involved in research & clinical audit within the clinical area of CKD/renal Assist in identifying all potential recipients for transplantation work these patients up for referral Participate in the transplant on call rota Work collaboratively within the multidisciplinary team and participate and participate in the induction of new staff to renal Administration of live vaccinations COMMUNICATIONS & WORKING RELATIONSHIPS Assesses, develops and implements specialist nursing care programmes with the aim of increasing awareness and recognition of CKD in all patients. Effectively manage the communication with members of the multidisciplinary team for the smooth transition along the CKD pathway Provides specialist advice to patients and their families/carers, and to staff / students of all disciplines. Provides and receives highly sensitive, distressing, complex or contentious information to patients, their relatives and members of the public with empathy and reassurance. For example, unexpected complication of CKD as a consequence of presenting acute illness or following routine elective intervention.
Communicates with a range of other staff of all disciplines, acting as a highly specialist resource including liaison with health care providers outside of the Trust regarding diagnosis and treatment advice. Develops skills to assess and interpret specialist information and conditions and takes appropriate action in conjunction with the nephrologists. Produces and presents reports as required Has a personal duty of care in relation to equipment and resources Plans, delivers and evaluates education and training programmes for all relevant disciplines of staff. Establish, develop and maintain effective working between clinicians , practitioners and managers within the speciality , across the trust and in the community Provides specialist training and education in informal and formal settings for nurses and other disciplines.
Provides teaching in practice to other staff and students through clinical supervision/facilitation Acts as a positive role model for other staff and students Teaches patients and their families/carers about managing own condition, including after-care. Ensure clinical practice developments are based on best available evidence. Undertake research and use core audit skills to enable the specialist team and other health professionals to improve quality of care Seek opportunities to present data at local/national meetings Undertake patient reviews with the nephrologists on a monthly basis Participates in transplant on call, for screening calls, receiving offers and arranging kidney transplants
