Job description

Great Western Medical Practice are looking for a Chronic Disease Administrator, to join their team in Aberdeen.

The post holder will co-ordinate the organisation and ongoing management of chronic disease clinics, in order to support the development and monitoring of specific chronic diseases for the Practice.

The Chronic Disease Administrator will abstract, analyse, translate and data enter to patient records whilst ensuring high standards of data quality.
Part of their responsibilities include dealing with enquiries, liaising with GPs and other healthcare professionals and participating in multi-disciplinary meetings.


The job holder will have to:

  • Work with GPs and Practice Nurses to ensure that chronic disease clinic protocols are developed and implemented in order to ensure smooth running of clinics in line with the Quality Outcome Framework.
  • Meet with GP’s and Practice Manager on a regular basis to review the management of chronic disease clinics and progress against quality and outcome Indicators
  • Liase with GPs, Practice Nurses and Pharmacist on a regular basis to ensure the accuracy and smooth running of the chronic disease registers.
  • Responsible for developing and managing the Patient Recall System for each chronic disease area
  • In line with Chronic Disease protocols, they ensure compliance with agreed timescales.
  • Co-ordinate the recall to specific Chronic Disease Clinics, ensuring that the patient receives appropriate notification of their appointment time.
  • Responsible for ensuring accurate input of all data in order to maintain high standard up-to-date Chronic Disease Registers.
  • Use knowledge, experience and judgement to extract relevant information from the correspondence and then code accurately within the patient record.
  • Responsible for ensuring SCIMP/contract recommended READ Codes are used when entering diagnoses on to the clinical record.
  • Run searches on Vision in order to identify patients for invite to various Clinics, in line with each Chronic Disease protocol.
  • Identify and call patients for review based on the practice protocols, using their own judgement to decide whether there is need for blood tests or similar to be carried out prior to clinic review.
  • Ensure that all relevant invites for attendance at clinics are communicated promptly to patients via letter, or telephone, in order to maximise efficient running of chronic disease clinics.
  • Develop and run clinical system searches as required to allow analysis and audit of patient information and enable monitoring of progress against QOF indicators

Therefore, the successful candidate must:

  • Be educated to Higher Grade level such as HNC or equivalent experience, including medical terminology and coding procedures
  • Have knowledge gained via on-the-job experience and in-house/external training equivalent to NVQ level 4.
  • Have previous experience working within a GP Practice will be an advantage
  • Be able to work accurately within agreed parameters and timescales
  • Be familiar with SCIMP recommended READ codes as well current guidance on Exception Coding.
  • Have excellent IT skills
  • Have excellent communication skills, both oral and written.
  • Have excellent interpersonal skills

Located in the Seafield area in the West of Aberdeen, Great Western Medical Practice delivers healthcare for 10,000 patients from the ground floor of a modern health centre.

The practice is made up of various healthcare professionals to meet extensive patient needs and offers a variety of telephone, face-to-face and video appointments between 8am and 6pm, Monday to Friday.

There is carpark available for patients and staff. Practice is close to amenities, including pharmacies, shops and local bus routes.
