Applications are invited from suitably qualified and experienced nurses to join our established NHS Continuing Healthcare team following retirement of the current post holder.
You will need to be a Registered Nurse (General) with significant knowledge and experience of NHS Continuing Healthcare. The successful applicant will be expected to deliver a lead co-ordination and case management role in the area of CHC and Complex care, with a working knowledge of CHC procedures/legislation and to act as a point of contact for advice to other professionals, patients and their representatives. Heywood, Middleton & Rochdale (HMR) ICP operate a Continuing Healthcare/Complex case service on behalf of the registered population of HMR. The post holder will be responsible for co-ordinating the process of determination of eligibility for NHS funded Continuing Healthcare within the parameters of the National Framework for NHS funded Continuing Healthcare for patients registered with HMR
The post holder will take a lead role in the identification of suitable care providers and will be responsible for ongoing review and reassessment of patients and commissioned packages of care. This will be with a view to determining continued eligibility and ensuring that the package of care continues to meet assessed needs.
