The CHC team are seeking enthusiastic experienced Registered nurses with a can do attitude to join our dynamic team.
This role will require you to undertake CHC / Fast Track assessments, Funded Nursing Care reviews and make eligibility decisions on patients for adults over 18 years of age with complex health needs including the assessment, review and monitoring of care for those whose health needs are funded via NHS Continuing Healthcare within the dedicated time scales. This will include retrospective and appeal cases.
The post holder will act as a Adults Senior Nurse Assessor for individual clients within Birmingham Solihull ICB who are receiving continuing health care funding to assess the needs of the client and their family to facilitate, plan and evaluate service provision to ensure the client achieves an optimum level of function and receives appropriate levels of care in the most suitable environment.
- To review any service, and care, commissioned in regards to quality and appropriateness to the individuals need, to ensure that the provider is delivering the requirements of the contract ensuring value for money in all matters relating to packages for continuing NHS healthcare.
- The post holder will work jointly with health, social care and independent sector colleagues to ensure care management processes are appropriate.
- To follow the process laid out in the National Framework for NHS Continuing Healthcare; to include organisation, co-ordination and participation in MDT eligibility recommendations to be verified by ICB.
- The post holder will be responsible for complex clinical caseload comprising of; Assessments, undertaking of review (3/12 CHC review unscheduled review and Fast Tracks and implementation on Personal Health Budgets and ongoing Case Management of patients funded by Continuing Healthcare (CHC).
- Ensure assessments conducted are robust, transparent and multi-agency (where required) for NHS Continuing Healthcare/NHS-funded Nursing Care.

- To support the seamless implementation of the National Framework for Continuing Healthcare including contribution to the development of training programmes and delivery plans as required to facilitate others learning.
- Within Birmingham and Solihull ICB provide advice, screen and coordinate assessments against the National Framework for Continuing Healthcare and Funded Nursing Care within Nursing Homes.
- To work with the Senior Nurses to process Funded Nursing Care and Continuing NHS Health Care Applications.
- Influence best practice by providing professional nursing advice on a range of physical and mental health nursing issues, which will require the individual to work in collaboration with partners and other professionals to ensure patients' health care needs are met appropriately and effectively.
- A strong business focus as well as clinical expertise is a key requirement for this role to ensure delivery of best possible care within available funding representing value for money.
