We're looking for a Care Coordinator to help coordinate care with patients within the home health care services industry. The successful candidate will be responsible for communicating with physicians to arrange appointments, communicating with patients and their families, as well as coordinating all aspects of the home health care process, including medical supplies and billing. Our Care Coordinator will be part of a team that is devoted to finding ways to provide the best care possible for our patients.

Salary: £24,000.00-£24,542.39 per year

Ability to commute/relocate:
