Job description
Please refer to the attached job description and person specification for full details of responsibilities As a Case Manager, the post holder will work as part of a dynamic team in delivering an effective service to ensure safe and effective care pathways are in place for all service users from across the East of England in specialised mental health care settings, within the NHS and Independent sectors. To provide clinical support to the specialist commissioners in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs. The role supports the business in driving transformation as well as value for money in planning, commissioning and service the role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery. Due to the role being across the East of England the nominated base can be negotiable.
