Job Description: Fixed term until 30 June 2024 | £51,791 to £56,984 per annum (Scale M) | 37 hours per week | County Hall, Norwich Norfolk County Council is committed to delivering a short-term offer that recovers, enables and assesses people who require short term care. As Business Lead for the Short-Term Offer Programme, you will play a key role in shaping and delivering the successful transformation of the short-term offer that supports people discharged from hospital to return home and regain their independence. You will be working alongside commissioners who are working across several sectors (including home support, residential/ nursing care, day services and the voluntary sector,) to develop services. You will also be expected to work with operational teams across the county to support implementation and work closely with partners in NHS commissioner and provider organisations, and other stakeholders as identified to provide leadership for our home first approach. You will have the opportunity work on both county-wide priorities and Place based commissioning. You will be responsible for developing and implementing the strategy to transform short term services in the community. Transformation of the short-term services offer has been identified as a priority for the department, and you will be developing significant recommendations on service design and spend. You will also be expected to work with a number of teams and partners, to deliver those changes to commissioned services and maximise the impact of this work.
