The Long Term Plan set out by NHS England in 2019 was to ensure every person diagnosed with cancer will have access to personalised care, including holistic needs assessment, a care plan and health and wellbeing information and support. Cancer Services and the newly established personalised care team are passionate and focused on improving training and education of staff working within cancer as well as a wider Trust. The Post Holder will work closely the Cancer Information and Support Services, Clinical Nurse Specialists Clinical Teams and Allied Heath Professionals, covering the patient phone line and will contribute to the organisation and delivery of health and well-being events. Main duties, tasks & skills required The post holder will form an integral part of the personalised care team to provide cancer patients with information and support that meets their individual needs. To discuss what matters to them and provide cancer information and/or signpost them to support services. Assist and promote cancer awareness campaigns and engagement events.
