Are you looking for a new, exciting challenge at work? The new Virtual Ward service is being developed to commence with a test phase and pilot phase from November 2022. This service will support patients with acute Frailty and/or Respiratory needs who will have their acute care provided in the community instead of having their care within a hospital setting.
The Virtual Ward service will offer acute care at home to Kirklees residents 7 days a week between the hours of 8am and 8pm, with some out of hours support to ensure our patients are safe. The patients will be supported across Kirklees by a dedicated team, with oversight and support from Acute Consultants / appropriate clinicians. As a key member of the Virtual Ward, you’ll be part of our continuing success in Kirklees, preventing admission to hospital and supporting earlier discharge by helping keep patients at home. The service will work as part of a Calderdale, Kirklees and Wakefield (CKW) approach.
The Virtual Ward service will be a multidisciplinary team with Locala colleagues and will include Frailty Nurses, Respiratory Nurses and Therapists, Health Care Support Workers and an Administrator. They will work in partnership with clinicians within the local Acute Trusts to ensure patients have a holistic and safe service offer.
