We work together across health, care, local authorities and the community and voluntary sector to meet the health care and wellbeing needs of our populations. Our ambition is that our population will start well, stay and live well, age well and die well.

We believe we can achieve more by working together. We want to go further joining up health and care services and considering all of a person’s health, wellbeing and social care needs. This means investing our collective resources with a focus on prevention, early intervention and responsive care we have a clear focus on the wider determinants of health and reducing health inequalities. Vital to our success are our relationships with local communities as we work to develop a culture of healthy living and supportive neighbourhoods.

Building on the strengths and experience gained as one of the first Integrated Care Systems in England Surrey Heartlands continues to lead national approaches to integration and systems leadership. We are looking for an exceptional leader to join our Guildford and WaverleyPlaceteam as we seek to implement the Integrated Care Strategy, working together as partners to best effect to support and meet the needs of our local populations.

This role is a leader in our system, we are looking for someone who has experience, learning and knowledge to share, someone who will bring curiosity and innovation in their approach.

Well developed partnership working skills will be essential as this role is pivotal in leading the development and delivery of collaborative transformation.

The role can be delivered through a mix of local face to face and remote virtual working across Guildford and Waverley and the Surrey Heartlands ICS.

The post holder will provide matrix leadership to the priority projects and programme work, forming trusted relationships across partner organisations and with residents and local communities and leaders.

As a member of our senior leadership team the post holder will have clear responsibilities to enable achievement of our Place based responsibilities including Surrey Heartlands Critical Five, implementing the findings of the Fuller Stocktake, key operational targets and our priority areas of focus.

Candidates will be required to attend a virtual stakeholder panel session prior to interview (date to be confirmed).
