This is a fixed term/secondment vacancy until 31/03/2024

An Assistant Practitioner Nursing is expected to independently manage their own work and case load. The post holder will take responsibility for the provision of person-centred care in accordance with organisational policy and procedures and will:

Be responsible for some elements of patient assessment, undertaken in conjunction with the registrant, implementing programmes of a care plan, and modifying programmes of care as delegated by a registered practitioner.

Report regularly to the registered practitioner, recognising own limitations and seeking advice if they are unsure.

Provide general/specific care as specified below for a specific client group and will be competent to work with other professional disciplines, referring patients for allied health professional assessment and treatment or to other agencies as required.

Delegate work to other unregistered staff within agreed protocols, and may supervise, teach and assess other un-registered staff.

The role will provide high quality and routine care management to families in their own homes, with the guidance and support of an experienced Health Visitor and the community public health nurses.

This includes liaising with staff and carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package. This role will require to facilitate and support groups in the community setting and be able to deliver the key public health messages, along with supporting families in their own home with parenting, such as Toilet Training, Breast Feeding, Sleep management, Behaviour management, Diet and Weaning and school readiness.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
