An exciting opportunity has become available for a Clinical Nurse Advisor in the Flow Centre. The Flow Centre operates a 24/7 centralised service to support the flow of patients across all adult acute sites in Lothian. The Clinical Nurse Advisors work with GPs, Health & Social Care Partnerships, NHS 24, the Scottish Ambulance Service and acute hospital staff to create clear pathways for admission and access to alternatives to admission. NHS Lothian’s Flow Centre has changed the way unscheduled and urgent care is delivered.
The current team of Clinical Nurse Advisors provides a clinical service 24/7. They receive referrals via NHS 24 and carry out a virtual consultation with the patient. Their role is to assess the urgency of the clinical presentation and to triage to the most appropriate service, signposting to alternative pathways and diverting patients away from hospital and front door areas. Approximately 30% of patients referred by NHS 24, Scottish Ambulance Service and health care professionals are now being treated either in the community setting or at an outpatient, same day service or Minor Injury clinic.
This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
