Youth Transition Family Support Worker
Salary:£22, 894 pro rata Hours:18.5 per week
Two year fixed contract

Include Me TOO (IM2) is a national charity championing disability rights and inclusion, social justice and equality for all disabled children and young people. We are an established B.A.M.E led disability service provider based in the heart of Wolverhampton in the West Midlands. We have a great opportunity a Youth Transition Family Worker role to join our dedicated team to empower and capacity build B.A.M.E and Marginalised communities' parents and carers of disabled children and young people. Providing access to advocacy for the disabled rights and service provision needs that will best support disabled children and young people to reach their full potential. This role will focus on supporting parents and carers of disabled children and young people during their child(ren)’s school transitions into adult provisions, Educational Health Care Plan (EHCP’s) assessments, and navigating the SEND system pathways.
