Hours: 30 Hours Per Week (Part Time)
Salary: £25,000-£26,906 Pro Rata. Salary dependent on skills, experience and knowledge.
Contract: Fixed Term - 6 Months
Base: Waterford House, Wokingham, with travel around Wokingham

What will you do?
The Berkshire Physical Health Checks Project will deliver physical health interventions across several primary care settings. This is to recognise the value of physical health as part of our client’s mental health recovery plan.
The project is embedded within other services (primary care networks). It will deliver to targets and provide support to colleagues to empower service users through person-centred physical health plans.
Two Physical Health and Wellbeing Workers will form a small team working closely with Primary Care Networks in Wokingham. They will safely deliver comprehensive physical health check assessments and review appointments for people on the SMI register and signpost them to appropriate services for ongoing support.
Key responsibilities
- Deliver physical health checks for people on the SMI register
- Provide ongoing support to service users to support them to make positive lifestyle changes
- Provide education around the importance of physical health checks to service users, families, carers and health care professionals
- Ensure timely, accurate reporting on databases
- Maintain and develop skills to achieve best practice utilising training and other development opportunities
- Work within policies and procedures of OXM and the different Wokingham PCN’s
- Be responsible for recording and reporting all safeguarding issues
- Liaise with partner organisations where required and develop these links
- Ensure that service users can engage and influence in the running of the service.
- Understand and implement all safeguarding policies and procedures in relation to working with children, young people and adults at risk of harm
- Enhance equity, diversity, inclusion and equality within our work
- Involve people with lived experience of mental health problems in every aspect of our work
- Support and supervise volunteers and peer supporters as agreed with your line manager
- Attend regular meetings and supervision as required and take reflective approach to their practice.

Essential Experience
a) Knowledge of Mental Health conditions and how these can affect individuals in a variety of ways
b) Knowledge of how mental health can impact physical health
c) An understanding of good practice in safeguarding Children and Adults
d) Experience of working within mental health, wellbeing or similar environments
e) Experience of working in a demanding environment
f) Experience of working as part of a team and communicating effectively
g) Experience of working with individuals or groups, enabling them to identify their own goals to support personal wellbeing and recovery, and helping them to pursue these.
h) Ability to work both collaboratively as a team and independently
i) Excellent listening and interpersonal skills
j) Ability to use confidently use Outlook, Word and Excel at a basic level.
k) Good planning and organisational skills

Desirable
a) Working knowledge of the range of services offered by Oxfordshire Mind and relevant local/national organisations

d) Understanding purpose of project and how their work meets the overall aims

f) Experience of identifying and managing a risk, with the support of a manager
g) Experience of supporting people with multiple/diverse needs
h) Experience of data collection and succinct, timely and accurate data entry to enable the measuring of outcomes and service impact

j) Experience of supporting/training staff or volunteers
k) Ability to utilise a range of tools and approaches to empower service users
l) Ability to effectively use safety planning tools in line with prevention of suicide and self-harm
m) Taking responsibility for own health and wellbeing to cope effectively with the pressures of the post and to use the support and supervision provided

p) Knowledge, understanding and evidence of ‘living’ the values in the practice carried out with Service Users, Peers and the wider organisation.

s) Enthusiasm and professionalism in representing Oxfordshire Mind to external organisations
Ability to travel between sites when required
Oxfordshire Mind is committed to safeguarding and promoting the welfare of children and vulnerable adults and expects all employees and volunteers to share this commitment.
