Merton Uplift offers primary care mental health provision for residents, incorporating a Wellbeing Service, the Merton IAPT service and a Primary Care Recovery Service.

The Merton Uplift Wellbeing Service builds resilience in the community and empowers local people to meet their physical, social and emotional needs. The service supports early intervention and community capacity building via community outreach, education and co-production initiatives. The service breaks downs barriers to accessing mental health support, and empowers people to be active, connect with and contribute to their communities. The team is full of energy and supportive colleagues, who enjoy collaboration and working creatively.

Due to career growth of colleagues, we are looking for a new Wellbeing Practitioner to join the team as the service moves forward with the ambition to place co-production with local communities at the heart of our service.

The Wellbeing Team undertakes targeted work to connect and partner with a range of groups and communities who have not traditionally accessed IAPT Services.

The post holder will support access to psychological therapies through the delivery of co-production, group workshop delivery and signposting in community and primary care settings. You will be using co-production and a strength based approach to empower and motivate those who access our services in partnership with our Wellbeing Partners: Carers Support Merton, Wimbledon Guild, Focus-4-1 and Polish Family Association.

Successful applicants will be passionate about improving the health and wellbeing of individuals as well as the wider community. You will be skilled in engaging a variety of stakeholders and be able to positively challenge stigma and discrimination using your excellent communication skills. You will have experience working directly with people in a health or social care setting and ideally have experience offering health information to groups or individuals.

  • To develop high standard interventions and group workshop content and resources in line with client and community need
  • To facilitate or co-facilitate interventions in a group or individual basis, arranging sessions, collating materials and resources as required
  • To risk assess and manage group dynamics both on site and in the community
  • To undertake protocol based psychological assessments of clients including self-report measures and rating scales
  • To promote and discuss wellbeing interventions with potential participants, staff and stakeholders to encourage attendance and engagement
  • Prepare for interventions and sessions including understanding material and appropriate delivery method given range of needs presented by group participants
  • Attend post intervention appraisals to review feedback forms and objectives
  • To maintain the highest standards of clinical record keeping and report writing in accordance with Trust policies and procedures and relevant professional codes of practice.
  • To assist the Wellbeing Lead and others in undertaking data collection, analysis and production of reports and summaries, in relation to primary care mental health services.
  • To edit, reflect and adapt intervention content in line with co-production discussions across community groups
  • To work with community members to develop specific and tailored interventions for explicit groups
  • To help research and draft marketing material for publication by the team, supporting other members of the team and working with managers across the service.
  • To assist in the implementation of service co-production projects as required.
