Job description
We are looking for four highly motivated professionals with excellent communication skills who are able to provide both strong advocacy on behalf of the client group and partnership working within the role. We are looking for someone who has a keen interest in the care of vulnerable womenunderstanding that the women of Croydon come with a vast diversity of complex needs. Providing continuity of carer will enable women with moderate to extremevulnerabilitiesto build a midwife-client relationship through the antenatal and postnatal periods whilst coordinating their multi-agency care.
Whilst working in this role you will be expected to work as a team player, your role will require detailed risk assessment skills, partnership working and a strong commitment towards achieving the outcomes that improve the health of all client groups. You will be expected to demonstrate clinical expertise and governance in all aspects of your work with particular attention to safeguarding maternity concerns being supported by robust specialist safeguarding supervision, guidelines and policies.
In this role your will:
- manage your caseload of women
- take on new referrals
- follow up on safeguarding concerns within the wide community teams
- perform home visits both antenatally and postnatally
- work in the wider multi-agency field, making necessary referrals when applicable
- attending core group meetings for your own caseload and supporting when your colleagues are unable to attend.
- supporting the general community midwifery staff to provide care to women with low level vulnerabilities within the traditional midwifery framework of care.
CUH provides more than 100 specialist services and is home to the borough’s only Emergency Department and 24/7 maternity services, including a labour ward, midwifery-led birth centre and the Crocus home birthing team.
Purley War Memorial Hospital (PWMH) in the south of the borough offers outpatient care, including diagnostic services, physiotherapy and ophthalmology services run by Moorfields Eye Hospital, alongside an onsite GP surgery.
Our experienced district nursing teams, Allied Health Professionals and community matrons look after for people of all ages across Croydon and our Children’s Hospital at Home cares for children with long-term conditions without them having to come to hospital.
