Hope House Children’s Hospices have a unique opportunity for a Transition Youth Worker to join our team based Oswestry. You will join us on a full-time, permanent basis working office hours. In return, our Transition Youth Worker will receive a competitive salary of £30,750 - £37,466 per annum. Which is equivalent salary of Band 5 to mid Band 6 within the NHS.
Our vision is to ensure that children affected by conditions so serious that they are not expected to reach early adulthood enjoy with their families the best quality of life – with access to specialist care and bereavement support when they need it.
About the Transition Youth Worker role:
We are seeking a skilled and dynamic individual who can effectively manage a caseload for young people with complex life limiting conditions on their journey into early adulthood within the Transition Service. You will work alongside the wider teams at Hope House and Tŷ Gobaith, and external care providers. You will empower the young people to have their own voice, and to enable them to have the confidence to navigate into the adult care, housing, education and benefits system.
What we are looking for a Transition Youth Worker who:
- Possesses outstanding communication and people skills, particularly with our young people who are non-verbal
- Is an excellent decision-maker with the ability to multitask and acute assessment skills
- Has a comprehensive understanding of the needs of children with disabilities, young people with life-limiting conditions and knowledge of children’s palliative care
- Has excellent knowledge Transition knowledge – Social Work or Nursing qualified along with comprehensive and up to date understanding of the charity, adult social health and care sectors
